AVALIAÇÃO DO DISTÚRBIO DA DEGLUTIÇÃO NOS PACIENTES COM PARACOCCIDIOIDOMICOSE NAS VIAS AÉRO-DIGESTIVAS SUPERIORES

Introdução: A paracoccidioidomicose (PCM) é a micose sistêmica de maior prevalência na América Latina, causada pelo fungo Paracoccidioides brasiliensis. A doença primária ocorre nos pulmões e pode disseminar-se para outros órgãos e tecidos, ocasionando lesões secundárias principalmente na mucosa bucal, que podem ser o primeiro sinal da doença. As lesões mucosas apresentam-se exulceradas e ulceradas, com superfície granulomatosa, de fundo amarelado, entremeadas por pontos hemorrágicos conferindo aspecto moriforme. Uma importante característica a ser considerada é a dor observada devido às lesões de PCM, podendo acarretar disfunções locais. A deglutição é uma ação automática, comandada pelo tronco encefálico, que serve para transportar o bolo alimentar para o estômago e realizar a limpeza do trato respiratório. Objetivo: Considerando a importância da deglutição para a manutenção da fisiologia normal do corpo humano, pretende-se descrever as alterações da deglutição decorrentes da PCM nas vias aéreo-digestivas superiores (VADS), associando-as ao sexo, idade, apresentação clínica da doença, tempo de evolução da doença e sintomas associados. Métodos: Foi realizado um estudo retrospectivo de em prontuários de 82 pacientes tratados de PCM das VADS e acompanhados no Instituto de Pesquisa Clínica Evandro Chagas, Fundação Oswaldo Cruz, no período de 2008-2013 Foram coletados dados sóciodemográficos, clínicos e terapêuticos. Foram testados três desfechos (presente/ausente) relacionados à deglutição: disfagia, odinofagia e emagrecimento. As variáveis categóricas foram associadas ao desfecho pelo teste qui-quadrado de Pearson e teste exato de Fisher, enquanto as variáveis contínuas foram comparadas aos desfechos pelo teste t, no caso paramétrico e, pelo teste de Mann-Whitney, no caso não-paramétrico. Resultados: A cavidade oral foi o principal sítio das VADS acometido por PCM, seguido da laringe, mucosa nasal e faringe. A odinofagia foi o sintoma mais frequente do acometimento das VADS por PCM, seguido por disfonia, disfagia, tosse e dispnéia. A odinofagia e a disfagia foram associadas entre si e à localização orofaríngea, laríngea e ao emagrecimento. O emagrecimento também foi associado ao maior tempo necessário de tratamento da PCM. Conclusão: A associação entre odinofagia e disfagia com emagrecimento, sugere que, na PCM a disfagia e a odinofagia decorrentes da presença de lesões mucosas nas VADS, levem a uma redução da ingestão alimentar e consequente emagrecimento, com a alteração do estado nutricional, levando ao maior tempo na cura das lesões
Abstract
Introduction: Paracoccidioidomycose (PCM) is the most frequent profound mycose in Latin America, caused by the fungus Paracoccidioides brasiliensis. The primary disease occurs in the lungs and can spread to other organ and tissue, making secondary lesions mostly on the oral mucosa, which can be the first sign of the disease. The mucosal lesions are present if an ulcerated, with granulous surface, with yellow base, with hemorragic points giving it an moriforme aspect. An important caracteristic to be consider is the pain on the PCM lesions may cause local malfunctions. Deglutition is an automatic action set off by the brainstem. It transports the food from the mouth to the stomach, from the pharynx to the esophagus. Considering the importance of deglutition to the maintnence of the normal physiology, there is the need to investigate the association betwen granulous disease and the superior digestive-airway system (SDAS) with deglutition disorders. Objective: Considering the importance of swallowing for maintenance of the normal physiology for the human body, this study intends to describe the changes of deglutition disorders caused by PCM on the SDAS, associating with gender, age, clinical presentation of the disease, time of evolution and associated symptoms Methods: It is a retrospective study with active search in medical records of 82 patients treated of PCM on the SDAS and followed at the Otorhinolaryngologic Clinic of IPEC-FIOCRUZ betwen 2008-2013. The interest variables were demographic data, clinical and theurapeutic. Were tested Thre outcomes (Present/absent) related to swallowing: dysphagia, odynophagia and weight loss. The cathegorical variables were associated did the chi-square test and Fisher exact test, whereas continuous variables were compared by t test for outcomes, in parametric case, and by the Mann-Whitney test in the non parametric. Results: The oral cavity was the main site of SDAS affected by PCM, followed by pharynx, larynx and nasal mucosa. Odinophagia was the most common symptom of involvement of the SDAS by PCM, followed by dysphonia, dysphagia, cough and dyspnea. Odinophagia and disphagia were associated betwen each other and the location, oropharyngeal, laryngeal, to weight loss. This last one was also associated to the longest PCM treatment. Conclusions: The association betwen odynophagia and dysphagia with weight loss hypotheses that on the PCM, dysphagia and odynophagia caused by the SDAS, causes a reduction on the food ingestion and therefore weight loss, with changes in nutritional status, causing the wounds to heal slower
