Este trabalho teve como objetivos: (i) avaliar o processo de controle da qualidade de vacinas contra febre amarela, utilizadas pelo Programa Nacional de Imunizações do Ministério da Saúde no período de 2000 a 2008, pelo levantamento de dados provenientes do Sistema de Gerenciamento de Amostras do Instituto Nacional de Controle de Qualidade em Saúde (INCQS) da Fundação Oswaldo Cruz(FIOCRUZ); e (ii) propor a utilização de gráficos de controle, como ferramentas úteis para a melhoria desse processo, pela análise da consistênciade produção e detecção de tendências sistemáticas. A análise relativa ao controle da qualidade das vacinas contra febre amarela foi constituída dos seguintes parâmetros: ensaios de (i.) potência; (ii.) termo estabilidade; (iii.) determinação de ovalbumina residual; (iv.) esterilidade bacteriana e fúngica; (v.) teor de umidade residual; (vi.) endotoxina bacteriana; e (vii.) análise do protocolo resumido de produção e controle emitido pelo fabricante. No período estudado, ingressaram no INCQS 1031 lotes de vacinas contra febre amarela, produzidos por dois fabricantes distintos (97 por cento - fabricante A e 3 por cento - fabricante B), representando um total de 285 milhões de doses individuais. O presente estudo mostra que no INCQS o processo está sob controle estatístico, e as causas especiais de variação, caso ocorram, são adequamente monitoradas.
Resumen en ingles
This study aimed: (i) at evaluating the process of quality control related to yellow fever vaccines used by the National Immunization Program (NIP) of the Brazilian Ministry of Health during the period from 2000 to 2008, by using survey data from the Sample Management System (SMS) of the National Institute for Quality Control in Health (INCQS) - Oswaldo Cruz Foundation (FIOCRUZ); and (ii) to propose the use of control charts, as useful tools for improving the processes of consistency of production and detection of systematic biases. The analysis to check the quality control of vaccines against yellow fever was comprised of the following parameters: (i) potency testing, (ii.) thermo stability, (iii.) residual ovalbumin contents (iv.) bacterial and fungal sterility assay, (v.) residual humidity contents, (vi.) bacterial endotoxin, and (vii.) vaccine production and respective quality control protocols stated by manufacturers. During the analyzed period, 1,031 yellow fever vaccine batches were analyzed by INCQS, which were produced by two manufacturers (97% by manufacturer A and 3% by manufacturer B), representing a total of 285 million individual doses. The present study indicates that at INCQS the process is under statistical control, and the specific causes of variation, if they eventually occur, are reasonably managed.
