Introdução: o surgimento da terapia antirretroviral de alta potência mudou a história da infecção pelo HIV, permitindo o aumento da sobrevida dos pacientes e a mudança da característica da doença de infecção aguda e letal para comorbidade crônica. A doença renal destes pacientes também mudou, tomando perfil crônico também. Objetivos: investigar a prevalência de disfunção renal, definida como TFGe menor que 60 ml/min e determinar os fatores associados Métodos: estudo de coorte, retrospectivo e transversal, através do cálculo da taxa de filtração glomerular estimada pela equação CKD-EPI avaliando dados demográficos e clínicolaboratoriais de pacientes do IPEC
Resultados: a prevalência foi 3,8%. Os fatores associados foram faixa etária maior que 50 anos, contagem de linfócitos T CD4+ menor que 350 cels/mm3 no momento da dosagem da creatinina, presença de diabete melito ou hipertensão arterial sistêmica, uso prévio de tenofovir, uso prévio de indinavir e uso corrente de lopinavir ou atazanavir. Conclusão: a prevalência encontrada foi baixa. Alguns antirretrovirais apresentam potencial nefrotóxico porém este parece ser manejável através da vigilância clínico-laboratorial estrita
Abstract
Introduction
: Highly active antiretroviral therapy (HAART) chan
ged the course of
HIV infection, allowing longer survival and altering the face of AIDS from an acute
lethal disease to a chronic condition. Kidney disease among those patients changed
at the same time, becoming also of a more chronic character.
Objectives
: to investigate the prevalence of kidney dysfunction, defined as eGFR of
less than 60 ml/min and to establish associated factors
Method
: We performed a retrospective cross
-
sectional study. We obtained eGFR
values by the CKD
-
EPI equation and analyzed demog
raphics and clinical and
laboratorial of our patients.
Results
: the prevalence found was 3,8%. Associated factors were age over 50 years
old, less 350 T CD4 linfocite cels/mm3 at the moment of creatinina measurement,
diabetes, hypertension, past tenofovir
use, past indinavir use, current use of
atazanavir or lopinavir.
Conclusion:
Prevalence was considered low. Some antiretroviral drugs showed
nephrotoxic potential but it appears to be acceptable under strict clinical and
laboratorial follw
