Abstract in Portuguese
Introdução: As mortes infantis representam um problema para a saúde pública, pois são
mortes precoces e na maioria das vezes podem ser evitadas. Os avanços tecnológicos e o
largo combate às doenças infecto-parasitárias fizeram com que a mortalidade infantil caísse
de forma significativa no Brasil nas últimas décadas. O principal componente responsável
pela mortalidade infantil é o neonatal. Objetivo: Identificar os fatores neonatais associados
ao desfecho óbito na primeira internação imediatamente após o nascimento. Metodologia:
Foi realizado um estudo transversal retrospectivo. Foram incluídos os nascidos vivos no
IFF que ficaram internados imediatamente após o nascimento em UTI neonatal, sendo
excluídos aqueles que permaneceram no alojamento conjunto após o nascimento e os que
nasceram em outras instituições e posteriormente foram transferidos para o IFF. As
informações foram retiradas do banco de dados disponíveis no IFF. As variáveis foram
analisadas inicialmente de forma descritiva e posteriormente através de um modelo de
Regressão Logística Múltipla objetivando identificar as que contribuíram
significativamente com a ocorrência do desfecho óbito. Uma análise complementar foi
dedicada aos RN’s que foram a óbito na sala de parto. Resultados: No presente trabalho,
no modelo final, quando foram ajustadas todas as variáveis do estudo que apresentaram
significância estatística, a análise multivariada mostrou aumento do risco de óbito
associado aos RN’s com extremo baixo peso ao nascimento, APGAR 5’ menor que 7,
fizeram uso de respirador, tiveram pneumonia, e apresentaram alguns tipos de
malformações congênitas. O uso de capacete como recurso de assistência respiratória
apareceu como fator protetor. Conclusão: O óbito neonatal foi associado ao peso menor
que 1000 gramas ao nascimento, APGAR no quinto minuto menor que 7, uso de respirador,
apresentar pneumonia e alguns tipos de malformações congênitas.
Abstract
Introduction: Child deaths represent a problem for public health, because they are early
deaths and most often can be avoided. Technological advances and fight off the infectious
and parasitic diseases caused infant mortality fell significantly in Brazil in recent decades.
The principal component is responsible for the newborn infant mortality. Objective: To
identify factors associated with neonatal outcome death at first hospitalization immediately
after birth. Methodology: We conducted a retrospective cross-sectional study. Live births
in the IFF who were hospitalized immediately after birth in the neonatal ICU were
included, and excluded those who stayed in rooming after birth and those born at other
institutions and were later transferred to the IFF. The information was taken from the
database available on IFF. Variables were initially analyzed descriptively and later by a
multiple logistic regression model aiming to identify who contributed significantly to the
occurrence of the outcome death. An additional analysis was devoted to RN's who died in
the delivery room. Results: In this work, in the final model when they were adjusted all
study variables that were statistically significant, multivariate analysis showed an increased
risk of death associated with RN's with extremely low birth weight, APGAR 5 'less than 7,
made use of respirator had pneumonia, and showed some types of congenital
malformations. Helmet use as a resource for respiratory assistance appeared as a protective
factor. Conclusion: The neonatal death was associated with weight less than 1000 grams at
birth, in the fifth minute APGAR lower than 7, mechanical ventilation, pneumonia and
present some types of congenital malformations.
