O Programa Saúde da Família (PSF) tem se configurado como um dos mais
importantes referenciais da organização da Atenção Básica no Brasil. A adscrição de
clientela, mencionada pelo programa como uma grande inovação desse modelo, tem
suscitado o debate sobre o conceito de território. Este artigo busca reaver o conceito
de território e suas relações com a saúde coletiva através de uma revisão bibliográfica,
bem como sua incorporação na prática de trabalho através da análise de documentos
oficiais do PSF. Foi possível observar que o entendimento sobre o conceito de
território varia nos âmbitos federal, estadual e municipal, assim como é diferenciado
dentro da própria Equipe de Saúde da Família. Partindo do PSF, a transformação da
realidade local está diretamente relacionada à multiterritorialidade, organizada através
da intersetorialidade dentro da Secretaria da Saúde, entre a Secretaria da Saúde e
outras Secretarias, e do poder municipal com a comunidade e outras instituições que
abrangem escalas geográficas diferenciadas. A delimitação de áreas e microáreas de
atuação, essencial para a implantação e avaliação do programa é, em geral, realizada
com base apenas no quantitativo de população, sem considerar a dinâmica social e
política, inerente aos territórios.
