Individuals with Tropical Spastic Paraparesis/ HTLV-I Associated Myelopathy (PET/HAM) usually report falls that may precede or show symptoms of this myelopathy and cause functional disability and morbidity, constituting a public health problem. This prospective cohort study has the overall objective of identifying the incidence of fall events in individuals with PET/HAM. It is understood that a reference instrument for the incidence of falls in this population would be a fundamental tool for planning effective fall prevention and rehabilitation strategies. When observing the literature, no studies were found on the incidence of falls in individuals with symptomatic PET/HAM and followed in a prospective cohort, which makes this research unprecedented. The work was carried out on the premises of the Instituto Nacional de Infectologia Evandro Chagas (INI/Fiocruz) The results showed that the estimated incidence of falls is similar to those found in other health conditions known to be associated with falls. The prevailing circumstances at the time when falls occur were also identified, as well as the predictive factors of these events and the risk of related injuries. The outcomes provide preliminary evidence that point to the need to implement strategies to mitigate the effects of falls in the population with PET/HAM. However, the context of social isolation generated by the COVID-19 pandemic may have contributed to the physical inactivity of the research participants and the consequent underestimation of the final data. Furthermore, social restrictions may have contributed to the small sample size, which limits the generalizability of the results
