A tuberculose (TB) é uma doença infectocontagiosa que apesar de ser muito antiga permanece como um problema de saúde pública mundial a ser enfrentado. A região de fronteira internacional entre Brasil e Argentina apresenta um grande fluxo populacional e concentração de grupos vulneráveis tornando o cenário favorável para a ocorrência da doença. Nosso objetivo foi analisar a distribuição espacial e a evolução temporal da incidência de tuberculose nos municípios pertencentes a região de fronteira internacional entre o estado de Santa Catarina, Brasil e da província de Misiones, Argentina no período de 2009 a 2021. Realizamos um estudo ecológico e estimamos a incidência da tuberculose que foi georreferenciada utilizando as bases de dados secundários dos sistemas de informação de notificação de casos dos dois países. Para detectar os padrões de distribuição espacial foram construídos mapas temáticos com as taxas médias de incidência de tuberculose brutas e padronizadas por idade, com as taxas suavizadas bayesiana empírica global e média móvel espacial. Na avaliação da autocorrelação espacial e identificação de Clusters espaciais foram calculados o Índice de Moran global e local. Para classificar a evolução temporal e verificar mudanças de tendência foram realizadas análises de regressão Joinpoint. Identificamos um padrão de distribuição espacial heterogêneo da TB, autocorrelação espacial positiva, presença de Clusters espaciais e tendência de crescimento de casos na região de estudo. Além disso, observamos que a faixa etária adulta e o sexo masculino são os mais acometidos pela doença. As áreas Norte, Noroeste e Sudoeste da região de estudo, localizadas na parte Argentina, concentraram a maioria dos casos, o que caracteriza uma área de elevada transmissão de TB, e a área Leste, situada na parte brasileira, apresentou as menores taxas de incidência de TB incluindo municípios silenciosos, o que chama a atenção, dentre outros, para a subnotificação de casos. Concluindo, devido a esta região de fronteira apresentar autocorrelação espacial positiva, presença de Clusters espaciais e tendência de crescimento de casos tuberculose, considera-se necessário para o controle da TB ações de todos os países que tem fronteira comum, como o aprimoramento das políticas públicas e dos indicadores da tuberculose, o firmamento de acordos bilaterais para financiamento e oferta de assistência em saúde, gestão compartilhada de sistemas de dados e programas de saúde pública, bem como serviços de vigilância em saúde integrados para assim alcançar efetivas medidas de promoção da saúde, mitigação e controle da propagação da tuberculose com uma abordagem específica e direcionada a situação da doença descrita neste estudo para cada localidade.

Tuberculosis (TB) is an infectious disease that, despite being very old, remains a global public health problem to be faced. The international border region between Brazil and Argentina has a large population flow and concentration of vulnerable groups, making it a favorable scenario for the disease to occur. Our objective was to analyze the spatial distribution and temporal evolution of the incidence of tuberculosis in municipalities belonging to the international border region between the state of Santa Catarina, Brazil and the province of Misiones, Argentina in the period from 2009 to 2021. We carried out an ecological and we estimated the incidence of tuberculosis, which was georeferenced using the secondary databases of the case notification information systems in both countries. To detect spatial distribution patterns, thematic maps were constructed with mean crude and age-standardized tuberculosis incidence rates, with global empirical Bayesian smoothed rates and spatial moving average. In the assessment of spatial autocorrelation and identification of spatial clusters, the global and local Moran Index were calculated. To classify the temporal evolution and verify changes in trends, Joinpoint regression analyzes were performed. We identified a heterogeneous spatial distribution pattern of TB, positive spatial autocorrelation, presence of spatial clusters and a growth trend in cases in the study region. Furthermore, we observed that the adult age group and males are those most affected by the disease. The North, Northwest and Southwest areas of the study region, located in the Argentine part, concentrated the majority of cases, which characterizes an area of high TB transmission, and the East area, located in the Brazilian part, presented the lowest incidence rates of TB including silent municipalities, which draws attention, among others, to the underreporting of cases. In conclusion, due to this border region presenting positive spatial autocorrelation, the presence of spatial clusters and a growing trend in tuberculosis cases, it is considered necessary to control TB actions from all countries that have a common border, such as improving public policies and tuberculosis indicators, the signing of bilateral agreements for financing and offering health assistance, shared management of data systems and public health programs, as well as integrated health surveillance services to achieve effective health promotion measures, mitigation and control of the spread of tuberculosis with a specific approach targeted at the disease situation described in this study for each location.
