AUDITORIA INTERNA NA ASSISTÊNCIA FARMACÊUTICA AMBULATORIAL COMO ESTRATÉGIA DE IDENTIFICAÇÃO E PREVENÇÃO DE ERROS DE DISPENSAÇÃO DE MEDICAMENTOS NO INI/FIOCRUZ

O ciclo da assistência farmacêutica compreende etapas de seleção, programação, aquisição, armazenamento, distribuição e dispensação de medicamentos e produtos para a saúde. O objetivo é a assistência segura ao paciente visando atender às suas necessidades mediante promoção da qualidade dos medicamentos dispensados, bem como dos serviços prestados. No Serviço de Farmácia do Instituto Nacional de Infectologia da Fiocruz (SEFARM - INI/Fiocruz), foram notificados erros de dispensação ambulatorial de medicamentos pelos profissionais da área e usuários. A reincidência dos erros provocou a reflexão sobre como criar barreiras que possibilitem os mitigar. Neste contexto, a Auditoria Interna é apresentada como estratégia de identificação proativa e prevenção de erros de dispensação de medicamentos aos pacientes ambulatoriais atendidos no INI/Fiocruz. O objetivo deste trabalho foi implantar Programa de Auditoria Interna como estratégia para a identificação de riscos e prevenção de erros de dispensação ambulatorial de medicamentos no INI/Fiocruz. A metodologia compreendeu quatro etapas: 1) Implantar Programa de Auditoria Interna, baseado nas diretrizes descritas na norma ABNT NBR ISO 19.011:2018, com formulação de Procedimento de Auditoria Interna – SEFARM e de Lista de Verificação para condução das auditorias. 2) Realizar auditoria interna na área de dispensação ambulatorial de medicamentos mediante aplicação de checklist cujo roteiro segue requisitos de normas técnicas referentes à dispensação de medicamentos aos pacientes ambulatoriais, os requisitos do Manual para OPSS (2022-2025) da Organização Nacional de Acreditação (ONA) e procedimentos internos 3) Realizar análise comparativa das não conformidades detectadas na auditoria interna com os erros de dispensação notificados pelos profissionais da área auditada e pacientes no ano de 2019. 4) Analisar criticamente os riscos detectados na auditoria, avaliando o impacto de cada um na etapa de dispensação e os hierarquizando, segundo os níveis da Matriz de Risco, descrita no Guia de Gestão de Riscos da Fiocruz. Como resultado da Auditoria, foi identificado elevado índice de não conformidades nos requisitos do Manual para OPSS (2022–2025) da ONA bem como no armazenamento de medicamentos na área de dispensação ambulatorial, ambos relacionados à inexistência de Sistema de Gestão da Qualidade implantado no SEFARM/INI. Como produtos deste trabalho, foram obtidos: O POP SEFARM 027 – Auditorias Internas no Serviço de Farmácia do INI e o checklist – Auditoria Interna na Dispensação Ambulatorial do INI. Após estabelecidos os níveis de risco inerente, observou-se como resultado um percentual elevado de riscos altos. Grande parte dos requisitos da auditoria interna foi focado na análise da sistemática de qualidade na dispensação ambulatorial de medicamentos, buscando práticas de controle e padronização de atividades, o que evidenciou as não conformidades inerentes a inexistência de Sistema de Gestão da Qualidade, refletindo em fragilidades na implantação de barreiras aos riscos na dispensação ambulatorial de medicamentos no INI/ Fiocruz. O estudo realizado reforça a importância de se implantar um Sistema de Gestão da Qualidade em Instituição prestadora de Serviços de Saúde
Abstract
The pharmaceutical care cycle comprises stages of selection, programmimg, acquisition, storage, distribution and dispensing of medicines and health products. The objective is to provide safe patient care in order to meet their needs by promoting the quality of the medicines dispensed, as well as the services provided. In the Pharmacy Service of the National Institute of Infectious Diseases of Fiocruz (SEFARM - INI/Fiocruz), errors in the outpatient dispensation of medicines were reported by professionals in the area and users. The recurrence of errors provoked reflection on how to create barriers that make it possible to mitigate them. In this context, Internal Audit is presented as a proactive identification and prevention strategy of medication dispensing errors to outpatients treated at INI/Fiocruz. The objective of this study was to implement in an Internal Audit Program as a strategy of identifying risks and preventing errors in outpatient drug dispensing at INI/Fiocruz. The methodology comprised four steps: 1) Implement the Internal Audit Program, based on the guidelines described in ABNT NBR ISO 19.011:2018 standard, with the formulation of the Internal Audit Procedure – SEFARM and the checklist for conducting audits. 2) Carry out an internal audit in the area of outpatient medication dispensing by applying a checklist whose script follows the requirements of technical standards regarding the dispensing of medications to outpatients the requirements of ONA Manual for OPSS (2022 – 2025) and internal procedures 3) Carry out a comparative analysis of the non-conformities detected in the internal audit with the dispensing errors reported by professionals in the audited area and patients in 2019. 4) Critically analyze the risks detected in the audit, evaluating the impact of each one in the dispensing stage and ranking them, according to the levels of the Risk Matrix, described at Fiocruz’s Risk Management Guide (FIOCRUZ, 2019). As results of the audit, a high rate of non-conformities was identified in the requirements of the ONA Manual for OPSS (2022–2025) as well as in the storage of medicines in the area of outpatient dispensing, both related to the lack of a Quality Management System implemented at SEFARM/INI. As products of this work, the following were obtained: POP SEFARM 027 – Internal Audits at the INI Pharmacy Service and the checklist – Internal Audit at INI Outpatient Dispensation. After establishing the levels of inherent risk, a high percentage of high risks was observed as a result. A large part of the internal audit requirements was focused on the analysis of the quality system in outpatient drug dispensing, seeking control practices and standardization of activities, which showed the non-conformities inherent in the lack of a Quality Management System, reflecting on weakness in the implementation of barriers to risks in outpatient drug dispensing at INI / Fiocruz. The study carried out reinforces the importance of implementing a Quality Management System in an Institution providing Health Services
