CONHECIMENTO DOS CUIDADORES DOS PACIENTES DE FIBROSE CÍSTICA SOBRE O MEDICAMENTO PANCREATINA EM UM HOSPITAL DE REFERÊNCIA NO RIO DE JANEIRO

Resumo
Introdução: A fibrose cística (FC) é uma doença de cunho genético, ocasionada por uma mutação no gene “cystic fibrosis transmembrane conductance regulator” (CFTR). O uso de medicamentos é um pilar essencial do tratamento para retardar a progressão desta doença e causar uma melhor qualidade de vida ao paciente. O tratamento consiste muitas vezes em diversos medicamentos, que incluem também classes farmacológicas distintas, entre eles, a pancreatina. Como na fibrose cística há a utilização de muitos medicamentos, é essencial para que haja uma avaliação de como os cuidadores de FC estão sendo orientados e a eficiência desse processo. Objetivo: Avaliar o conhecimento dos cuidadores de pacientes com fibrose cística do atendidos ambulatorialmente sobre o medicamento pancreatina em um hospital de referência no Rio de Janeiro. Método: Trata-se de um estudo observacional com caráter analítico e descritivo, com o intuito de medir o conhecimento dos cuidadores dos pacientes de Fibrose Cística sobre o medicamento pancreatina em um hospital de referência no Rio de Janeiro. Para atender aos objetivos específicos do estudo serão realizadas diferentes etapas. A coleta de dados será realizada pelo pesquisador durante os meses de Junho até Outubro de 2023 na farmácia ambulatorial do instituto estudado, às quartas-feiras de 08:00 às 12:00h. Para atender o primeiro objetivo específico, avaliar o conhecimento dos cuidadores acerca do medicamento pancreatina, será aplicado o questionário Conhecimento do Cuidador sobre Os Medicamentos (CPM-C) aplicado para os cuidadores de pacientes que tem os seus medicamentos dispensados pela farmácia ambulatorial do instituto estudado. Para atingir o segundo objetivo específico, identificar quais fatores podem interferir no conhecimento cuidadores de pacientes com fibrose cística sobre o medicamento pancreatina, serão realizadas entrevistas estruturadas utilizando o Roteiro de Entrevista Sociodemográfico. Resultados: Foram entrevistados 16 cuidadores, dos quais 10 (62,5%) deles eram mulheres, a idade predominante de 44 a 49 anos (31,25%), de escolaridade majoritariamente de ensino médio completo (37,5%) e maior parte eram cuidadores de pacientes que utilizavam a pancreatina entre dois a cinco anos (43,75%). Sobre o conhecimento dos cuidadores sobre o medicamento, a maior parte teve conhecimento insuficiente sobre a pancreatina (50%), as perguntas sobre processo de uso tiveram melhores médias de nota e as sobre segurança as menores. A pergunta com a maior porcentagem (94%) de respostas incorretas foi sobre precauções e a com maior número de respostas corretas (88%) foi a de duração do tratamento. Conclusão: Foi identificado que existe uma lacuna no conhecimento dos cuidadores sobre o medicamento pancreatina, visto que maior parte resultado de conhecimento insuficiente e ausente. Além disso, foi identificado um déficit maior no conhecimento das perguntas sobre precaução, contraindicações e reações adversas. Não foi identificada relação direta entre conhecimento dos cuidadores sobre o medicamento e escolaridade e tempo de uso de pancreatina.

Introduction: Cystic fibrosis (CF) is a genetic disease caused by a mutation in the cystic fibrosis transmembrane conductance regulator gene (CFTR). The use of medication is an essential pillar of treatment to slow the progression of this disease and provide a better quality of life for the patient. Treatment often consists of several drugs, which also include different pharmacological classes, including pancreatin. As in Cystic fibrosis there is the use of many drugs, it is essential for there to be an assessment of how CF caregivers are being guided and the efficiency of this process. Objective: To assess the knowledge of caregivers of patients with cystic fibrosis treated at an outpatient clinic about the drug pancreatin in a reference hospital in Rio de Janeiro. Method: This is an observational study with an analytical and descriptive character, with the aim of measuring the knowledge of caregivers of Cystic Fibrosis patients about the drug pancreatin in a reference hospital in Rio de Janeiro. To meet the specific objectives of the study, different stages will be carried out. Data collection will be carried out by the researcher during the months of June to October 2023 at the of the studied institute outpatient pharmacy, on Wednesdays from 08:00 to 12:00h. To meet the first specific objective, to evaluate the caregiver’s knowledge about the drug pancreatin, the Caregiver's Knowledge of Medications questionnaire will be applied to caregivers of patients who have their medications dispensed by the of the studied institute outpatient pharmacy. To achieve the second specific objective, to identify which factors may interfere with the knowledge of caregivers of Cystic fibrosis patients about the drug pancreatin, structured interviews will be conducted using the Sociodemographic Interview Script. Results: Sixteen caregivers were interviewed, of whom 10 (62.5%) were women, predominantly aged 44 to 49 years (31.25%), with mostly completed high school education (37.5%), and most were caregivers of patients who had been using pancreatin for two to five years (43.75%). Regarding caregivers' knowledge on the medication, the majority scored insufficient knowledge on pancreatin (50%), questions about the process of use had higher average scores, while those about safety had lower scores. The question with the highest percentage (94%) of incorrect answers was about precautions, and the one with the highest number of correct answers (88%) was about the treatment duration. Conclusions: It was identified that there is a gap in caregivers' knowledge about the medication pancreatin, as most resulted in classifications of "insufficient" or "does not know the medication". Furthermore, a greater deficit was identified in knowledge about questions regarding precautions, contraindications, and adverse effects. No direct relationship was identified between caregivers' knowledge on the medication and their education level or duration of pancreatin use.
