Introdução: A profilaxia pré-exposição (PrEP) oral com emtricitabina/tenofovir (FTC/TDF) é altamente eficaz na prevenção da infecção pelo HIV. O objetivo do estudo foi avaliar a descontinuação de seguimento/abandono de PrEP e os fatores associados entre travestis/mulheres transgênero (mulheres trans) e homens gays, bissexuais e demais homens que fazem sexo com homens (HSH). Metodologia: Estudo de coorte incluiu mulheres trans e HSH que iniciaram PrEP oral com FTC/TDF no Instituto Nacional de Infectologia Evandro Chagas (INI/Fiocruz) no período de 2014 a 2020. Descontinuação de seguimento/abandono de PrEP foi definida como não ter comparecido a qualquer visita dentro de 180 dias a partir da primeira dispensação de PrEP. As variáveis de exposição foram idade, gênero, raça, escolaridade; sexo transacional, sexo anal receptivo sem preservativo, ambos nos últimos 6 meses; binge drinking, uso de substâncias, ambos nos últimos 3 meses e sífilis na inclusão. Os participantes foram agrupados de acordo com o estudo ou programa no qual iniciaram PrEP (PrEP Brasil, PrEParadas, ImPrEP, PrEP SUS). Comparação entre grupos foi realizada através de testes Chi quadrado para variáveis categóricas e Kruskal-Wallis Ranksum para variáveis contínuas. Aplicamos um modelo de regressão logística multinível com interceptos aleatórios e inclinações fixas para identificar a associação entre a variável de desfecho e as variáveis de exposição. Resultados: Um total de 1.463 participantes (PrEP Brasil:12,3%; PrEParadas: 7,4%; ImPrEP: 43,5%; PrEP SUS: 36,8%) foram incluídos no estudo, com mediana de idade 29 anos [intervalo interquartil (IQR) 24 - 36], 83,4% eram HSH, 16,6% mulheres trans; 37,0% eram brancos, 24,3% negros e 37,2% pardos; 70,2% tinham concluído o ensino médio ou ensino superior (≥12 anos), 15,1% relataram sexo transacional; 58,6% relataram sexo anal receptivo sem preservativo e 15,2% tiveram diagnóstico de sífilis na inclusão. A descontinuação de seguimento/abandono da PrEP foi de 9,7% nos primeiros 180 dias de seguimento. Participantes mais jovens (18 - 24 anos) (razão de chance ajustada [aRC] 1,9, intervalo de confiança [IC] 95% 1,2 - 3,2), mulheres trans (aRC 2,8, IC 95% 1,6 - 4,8) e indivíduos com < 12 anos de escolaridade (aRC 1,9, IC 95% 1,2 - 2,9) tiveram maiores chances de descontinuar o seguimento/abandonar PrEP. Conclusão: Mulheres trans, indivíduos jovens e com menor escolaridade têm maior risco de descontinuar o seguimento/abandonar PrEP. Nossos resultados sugerem que o desenvolvimento de estratégias específicas para essa população deve ser uma prioridade
Abstract
Introduction: Oral pre-exposure prophylaxis (PrEP) with emtricitabine/tenofovir (FTC/TDF) is highly effective in preventing HIV infection. This study allowed to identify factors associated with PrEP early loss to follow up (ELFU) between travestis/transgender women (TGW) and gay, bisexual and other men who have sex with men (MSM). Methodology: This is s a prospective observational cohort study, which evaluated individuals who started oral PrEP at the Evandro Chagas National Institute of Infectious Diseases/Fiocruz (INI/Fiocruz) from 2014 to 2020. PrEP early loss to follow up (ELFU), was defined as not attending to any visit within 180 days from first PrEP dispensation. The exposure variables included age, gender, race, education; transactional sex, condomless anal intercourse [CAI] both in the past six months; binge drinking and substance use both in the past three months and syphilis diagnosis at baseline. Distributions were compared using chi-square test and Kruskal-Wallis Ranksum statistics, as appropriate. Multilevel logistic regression models with random intercepts and fixed slopes were used to identify factors associated with PrEP ELFU while accounting for the clustering of participants in each group (PrEP Brasil, PrEParadas, ImPrEP and PrEP SUS). Results: 1.463 participants (PrEP Brasil: 12.3%; PrEParadas: 7.4%; ImPrEP: 43.5%; PrEP SUS: 36.8%) were included in this analysis, the median age was 29 years (interquartile range [IQR] 24 – 360), 83.4% were MSM, 16.6% TGW; 37% auto identified as white, 24.3% as black and 37.2% as mixed/pardo; 70.2% completed secondary school (≥12 years). Transactional sex was reported by 15.1%; 58.6% reported CAI; 66.9% reported binge drinking, 33.1% reported substance use and, 15.2 % had syphilis diagnosis at screening. Overall, 137 participants (9.7%) met the definition of ELFU. Younger age (18 - 24 years) (adjusted odds ratio [aOR] 1.9, confidence interval 95%[CI] 1.2 - 3.2), being a TGW (aOR 2.8, 95% CI 1.6 - 4.8), having lower education (aOR 1.9, 95% CI 1.2 - 2.9) were associated with greater odds of ELFU. Conclusion: TGW, young individuals and those with lower education are at higher risk of PrEP ELFU. Our results suggest that the development of specific strategies targeting this population should be a priority, through policies that aim to reduce the incidence of HIV infection
