A hemorragia subaracnóide (HSA) aneurismática é um evento vascular arterial intracraniano agudo, decorrente a ruptura de aneurisma e sangramento abrupto, com alta mortalidade e complicações infecciosas, além de importantes sequelas neurológicas entre os sobreviventes. A letalidade varia amplamente, entre 8% e 67% em estudos epidemiológicos. Durante a hospitalização, os pacientes são susceptíveis a muitas complicações médicas que incluem, febre, insuficiência respiratória, disfunção cardíaca, tromboembolismo venoso, isquemia cerebral tardia, hidrocefalia dentre outras. Essas graves complicações impõem medidas invasivas aumentando o risco a infecções nosocomiais. Em países de alta renda, estas ocorrem em 26% a 41% e são um preditor independente de mal prognóstico. Dentre essas infecções, a ventriculite associada a cateteres de derivação ventricular externa (DVE) e a pneumonia associada a ventilação mecânica (PAV) são as que mais podem impactar o desfecho dos pacientes Nos países de média e baixa renda, incluindo o Brasil, há poucos dados epidemiológicos com um número significativos de pacientes e em mais de um centro de referência no tratamento da hemorragia subaracnóidea aneurismática. O acesso a dados mais precisos sobre a patologia em questão poderá ajudar o desenvolvimento de políticas públicas mais adequadas, além de abrir campo para novos estudos clínicos. O Instituto Estadual do Cérebro Paulo Niemeyer (IECPN) e o Hospital Cristo Redentor são unidades de referência para com HSA aneurismática, e recebem pacientes de toda a rede de saúde pública do Estado do Rio de Janeiro e do Estado do Rio Grande do Sul, respectivamente. Dessa forma, nós propomos um estudo prospectivo observacional multicêntrico com objetivo de definir a incidência de ventriculite associada a DVE e a PAV em pacientes com hemorragia subaracnóidea aneurismática e o desfecho neurológico hospitalar e de longo prazo

Aneurysmal subarachnoid hemorrhage (SAH) is an acute intracranial arterial vascular event, resulting from aneurysm rupture and abrupt bleeding, with high mortality and infectious complications, in addition to important neurological sequelae among survivors. Mortality rates varies widely, between 8% and 67% in epidemiological studies. During hospitalization, patients are susceptible to many medical complications that include fever, respiratory failure, cardiac dysfunction, venous thromboembolism, delay cerebral ischemia, hydrocephalus, among others. These serious complications impose invasive measures, increasing the risk of nosocomial infections. In high-income countries, complications occur in 26% to 41% and are independent predictor of poor prognosis. Among these infections, ventriculitis associated with external ventricular shunt (EVD) catheters and ventilatorassociated pneumonia (VAP) are those that can most impact the outcome of patients In low- and middle-income countries, including Brazil, there are few epidemiological data with a significant number of patients and in more than one reference center for the treatment of aneurysmal subarachnoid hemorrhage. Access to more accurate data on the pathology in question may help the development of more appropriate public policies, in addition to opening the field for new clinical studies. Instituto Estadual do Cérebro Paulo Niemeyer (IECPN) and Hospital Cristo Redentor are reference units for aneurysmal SAH, and receive patients from the entire public health network of the State of Rio de Janeiro and the State of Rio Grande do Sul, respectively. Therefore, we propose a prospective observational multicenter study with the objective of defining the incidence of ventriculitis associated with EVD and VAP in patients with aneurysmal subarachnoid hemorrhage and the in-hospital and long-term neurological outcome
