CONDUÇÃO DA REDE CEGONHA NO NÍVEL REGIONAL EM PERNAMBUCO: UM OLHAR SOB A PERSPECTIVA DA GOVERNANÇA REGIONAL

Resumo
O objetivo da presente estudo foi analisar a condução da Rede Cegonha (RC) em três Regiões
de Saúde do estado de Pernambuco, sob a perspectiva analítica da governança regional
exercida por meio da Comissão Intergestores Regional (CIR). Trata-se de um estudo de caso,
de abordagem prioritariamente qualitativa, utilizando como fontes de dados: documentos
institucionais e entrevistas semiestruturadas com gestores/gerentes. A investigação indica que
o exercício da governança regional por meio da CIR apresenta-se incipiente devido, em parte,
a baixa autonomia que os atores locorregionais apresentam sobre os recursos disponíveis para
as políticas de saúde. Dessa forma, posicionando a CIR como uma instância de ordem
burocrática com pontuais atuações para além das demandas da Secretaria Estadual de Saúde e
Ministério da Saúde. Cenário este também presente no âmbito da condução locorregional da
RC. O disparador das discussões para sua implantação no cenário estadual foram os atores
estaduais, influenciando na organização da rede regional a partir dos hospitais regionais de
administração indireta do ente estadual. Na CIR o processo de discussão da Rede Cegonha
têm ocorrido de forma fragmentada em“pastas setoriais”, com destaque a organização dos
serviços de média e alta complexidade para atenção ao parto e nascimento, em detrimento dos
demais componentes que atribuem o caráter de rede de atenção a saúde aos serviços do
território, como o sistema logístico e a articulação entre os serviços. A atenção básica,
manteve-se como uma atribuição de execução e monitoramento restrita ao ente federado
municipal sem discussões mais amplas em um contexto de planejamento regional. Essa
confluência de cenários infere a necessidade de-se repensar a exequibilidade de uma política
federal a ser estruturada nos heterogêneos cenários político-estruturais locorregionais,
perpassando por interesses diversos e se materializando de forma a reproduzir as lacunas no
qual se propôs a dirimir.

The objective of the present study was to analyze the conduction of the Rede Cegonha (CR)
in three Health Regions of the state of Pernambuco, under the analytical perspective of
regional governance exercised through the Regional Interagency Commission (CIR). It is a
case study, with a qualitative approach, using as data sources: institutional documents and
semi-structured interviews with managers / managers. The research indicates that the exercise
of regional governance through the CIR is incipient due in part to the low autonomy that
locoregional actors present over the resources available for health policies. Thus, positioning
the CIR as a bureaucratic order with specific actions beyond the demands of the State
Department of Health and Ministry of Health. This scenario is also present in the scope of RC
locoregional driving. The triggers of the discussions for its implementation in the state
scenario were the state actors, influencing the organization of the regional network from the
regional hospitals of indirect administration of the state entity. In the CIR, the process of
discussion of the Rede Cegonha has occurred in a fragmented way in "sectoral folders", with
emphasis on the organization of medium and high complexity services to care for childbirth
and birth, to the detriment of the other components that attribute the network nature of
attention to health services to the territory, such as the logistics system and the articulation
between services. The basic attention has remained as an attribution of execution and
monitoring restricted to the municipal federated entity without broader discussions in a
context of regional planning. This confluence of scenarios infers the need to rethink the
feasibility of a federal policy to be structured in the heterogeneous locoregional political-
structural scenarios, permeating diverse interests and materializing in order to reproduce the
gaps in which it was proposed to resolve.
