INTRODUÇÃO: O Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) é um betacoronavírus pertencente à família Coronaviridae. A Coronavirus disease-2019 (COVID-19), doença causada pelo vírus, apresenta quadro clínico inespecífico e de gravidade variada. O diagnóstico padrão da fase aguda é através de ensaios moleculares, no entanto, imunoensaios são utilizados o para rastreamento de casos. Dentre as proteínas do SARS-CoV-2, a fração RBD da proteína Spike consiste em um potencial candidato para ser usado como matriz antigênica em ensaios sorológicos indiretos. OBJETIVO: Avaliar o desempenho diagnóstico da fração RBD da proteína Spike do SARS-CoV-2 para o imunodiagnóstico da COVID-19. MATERIAL E MÉTODOS: A fração recombinante RBD foi cedida pelo Instituto de Desenho de Proteínas (Institute for Protein Design, Seattle-WA, EUA). O ELISA indireto foi padronizado para determinação da melhor quantidade do antígeno RBD, e a melhor titulação do anticorpo secundário IgG e IgM e das amostras séricas. A avaliação do potencial diagnóstico da RBD foi realizada utilizando 705 amostras séricas caracterizadas através de testes comerciais para detecção da infecção pelo SARS-CoV-2, sendo 354 amostras positivas para COVID-19 e 351 negativas, obtidas de doadores de sangue antes do início da pandemia. Para a avaliação da reatividade cruzada foram utilizadas 135 amostras pré-pandemia positivas para outras doenças infecto-parasitárias RESULTADOS: Após a definição das melhores condições para realização dos imunoensaios, o RBD-ELISA IgG apresentou boa capacidade diagnóstica com AUC de 84,6%, sensibilidade de 48%, especificidade de 99,1% e acurácia de 73,5%. Já o RBD-ELISA IgM demostrou capacidade diagnóstica razoável com AUC de 76,5%, sensibilidade de 51,1%, especificidade de 94,6%, e acurácia de 72,8%. Ao estratificar as amostras pelo intervalo de coleta após início dos sintomas, foram observadas diferenças nos parâmetros de desempenho para ambos os testes, sendo o intervalo de 15-21 dias com o melhor desempenho comparado aos demais intervalos. Ao avaliar a reatividade cruzada, o RBD-ELISA IgG obteve uma taxa de 4,4% (6/135) de reatividade, enquanto o RBD-ELISA IgM apresentou taxa de 3,7% (5/135). CONCLUSÃO: O RBD-ELISA IgG e IgM são capazes de diferenciar amostras positivas de negativas, apresentando boa capacidade diagnóstica. Quando realizada a estratificação dos resultados por data da coleta após a data do início dos sintomas, ambos os testes apresentaram elevado desempenho no intervalo de 15-21 dias. Todavia, o uso dos testes em paralelo possibilitou o aumento nos parâmetros de desempenho independente do período em que a coleta da amostra foi realizada
Abstract
INTRODUCTION: Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) is a betacoronavirus belonging to the Coronaviridae family. Coronavirus disease-2019 (COVID-19), a disease caused by the virus, has a nonspecific clinical picture of varying severity. The standard diagnosis of the acute phase is through molecular assays; however, immunoassays are used for case screening. Among the SARS-CoV-2 proteins, the RBD fraction of the Spike protein is a potential candidate to be used as an antigenic matrix in indirect serological assays. OBJECTIVE: To evaluate the diagnostic performance of the RBD fraction of the Spike protein of SARS-CoV-2 for the immunodiagnosis of COVID-19. MATERIAL AND METHODS: The recombinant RBD fraction was provided by the Institute for Protein Design, Seattle-WA, USA. The indirect ELISA was standardized to determine the best amount of RBD antigen, and the best titer of secondary antibody IgG and IgM and serum samples. The assessment of the diagnostic potential of RBD was performed using 705 serum samples characterized through commercial tests for the detection of SARS-CoV-2 infection, 354 of which were positive for COVID-19 and 351 negatives, obtained from blood donors before the start of the pandemic. For the evaluation of cross-reactivity, 135 pre-pandemic samples positive for other infectious and parasitic diseases were used RESULTS: After defining the best conditions for carrying out the immunoassays, the RBD-ELISA IgG showed good diagnostic capacity with an AUC of 84.6%, sensitivity of 48%, specificity of 99.1% and accuracy of 73.5%. The RBD-ELISA IgM, on the other hand, demonstrated a reasonable diagnostic capacity with an AUC of 76.54%, sensitivity of 51.1%, specificity of 94.6%, and accuracy of 72.8%. When stratifying the samples by the collection interval after the onset of symptoms, differences were observed in the performance parameters for both tests, with the interval of 15-21 days having the best performance compared to the other intervals. When evaluating cross-reactivity, the RBD-ELISA IgG had a reactivity rate of 4.4% (6/135), while the RBD-ELISA IgM had a rate of 3.7% (5/135). CONCLUSION: The RBD-ELISA IgG and IgM are able to differentiate positive from negative samples, presenting good diagnostic capacity. When the results were stratified by collection date after the date of symptom onset, both tests showed high performance in the 15–21-day interval. However, the use of tests in parallel allowed the increase in performance parameters regardless of the period in which the sample collection was performed
