Introdução: A Leucemia Linfoblástica Aguda de Células T (LLA-T) corresponde a 15% das LLAs da infância. Em países de alta renda, são observadas taxas de sobrevida livre de eventos (SLE) de 80 a 85%, já em países de média e baixa renda, são observadas taxas menores, com sobrevida global (SG) variando, entre 45 a 70%, o que reforça a necessidade de melhoria no tratamento nestes países. Na última década muito conhecimento vem sendo adquirido sobre alterações oncogenéticas e expressão de moléculas identificadas por imuno fenotipagem nas LLA-T, como biomarcadores prognósticos, embora ainda não haja consenso. Estudos mais recentes têm avançado na consolidação de alterações oncogenéticas como biomarcadores de prognóstico. Já a análise de Doença Residual Mensurável (DRM) por Citometria de Fluxo Multiparamétrica (CFM), realizada durante o tratamento, vem se consolidando como melhor fator prognóstico da atualidade. Aspectos clínicos-laboratoriais, como resposta ao tratamento inicial com prednisona (resposta no D8), e hiperleucometria acima de200 x 109/L, também vem sendo discutidos como marcadores de prognóstico. Objetivo principal: Avaliar a correlação dos resultados de DRM e sobrevida de pacientes de 0 a 19 anos com LLA-T tratados em centros de referência oncologia pediátrica na Bahia, com características clínico-demográficas, laboratoriais e imuno fenotípicas. Métodos: Foram estudados 86 pacientes com o diagnóstico de LLA-T, com idade entre 2 e 19 anos, nos 4 Centros de Referência em tratamento de câncer infantil na Bahia, de 2011 a 2020. Foram utilizados esquemas terapêuticos baseados nos Protocolos Berlin-Frankfurt-Muenster (BFM) 2002 e 2009, Protocolo do Grupo Brasileiro de Tratamento de Leucemias na Infância (GBTLI) 99 e Protocolo Recife para Leucemia Linfoblástica Aguda de células T 2011 (RELLA T 2011) Os dados foram obtidos através de busca em prontuários físicos ou eletrônicos. Resultados: No período selecionado não foram observados pacientes menores de dois anos, tendo sido encontrados 37 (51,4%) pacientes entre 2 e 9 anos, 27 (37,5%) pacientes entre 10 e 15 anos e 8 (11%) maiores de 15 anos. Com base nos protocolos de tratamento, 18% dos pacientes foram classificados no risco intermediário e 82% no alto risco. As taxas de SG e sobrevida livre de progressão (SLP) em 5 anos foram de 48% e 43,6%. Idade maior que 15 anos e a presença de adenomegalias se mostraram como variáveis independentes para análises de SG. A avaliação de DRM por CFM foi realizada somente a partir de 2014. Ao todo foram avaliados 52 pacientes: meio da indução (D15-19), final da indução (D29-33) ou consolidação da indução (D64-78), sendo analisados, 35, 44 e 24 pacientes, respectivamente. No entanto, somente 16 pacientes foram avaliados nos 3 momentos, o que dificultou as análises estatísticas. Na avaliação dos óbitos foi observado que 26,7% dos pacientes faleceram em remissão, entre os quais a principal causa de óbito foi infecção (91,7%), já 53,3% faleceram com doença ativa e em 20% não foi possível avaliar o status da doença. A ausência de adenomegalias ao diagnóstico foi identificada como fator prognóstico com impacto na melhoria da SLP. Nas correlações de desfechos com os marcadores imuno fenotípicos, a positividade de CD11b e HLA-DR foi associada a uma menor taxa de SG, por outro lado, a presença do CD8 foi associada a uma melhor SG. Conclusões: As avaliações de sobrevida e causas dos óbitos na população estudada indicam a necessidade de melhorias no tratamento, principalmente na terapia de suporte. Foram identificados achados imuno fenotípicos com impacto positivo (CD8) e negativo (CD11b e HLA-DR) na sobrevida global Tais achados devem ser investigados em trabalhos prospectivos e multicêntricos, pois trata-se de um exame acessível em nosso meio e com possíveis benefícios para a classificação de risco das LLA-T
Abstract
Introduction: T-Cell Acute Lymphoblastic Leukemia (T-ALL) accounts for 15% of childhood ALL. In high-income countries, event free survival (EFS) rates of 80-85% are observed, while in middle and low-income countries, lower rates are observed, with overall survival (OS) ranging from 45 to 70%, which reinforces the need for improvement in treatment in these countries. In the last decade, much knowledge has been acquired about oncogenetic alterations and expression of molecules identified by immunophenotyping in T-ALL, but there is currently no consensus on the use of this information as prognostic biomarkers. On the other hand, the analysis of Measurable Residual Disease (MRD) by Multiparametric Flow Cytometry (CFM) has been consolidated as the best prognostic factor nowadays. Objective: To evaluate the correlation of MRD results and survival of patients aged 0 to 19 years with T-ALL treated at reference pediatric oncology centers in Bahia, with clinical-demographic, laboratory and immunophenotypic characteristics. Methods: We studied 86 patients diagnosed with ALL-T, aged between 2 and 19 years old, at the 4 Reference Centers specialized in the treatment of childhood cancer in Bahia, from 2011 to 2020. Therapeutic regimens were used, based on the Berlin-Frankfurt-Muenster (BFM) 2002 and 2009, on the Protocol of the Brazilian Group for the Treatment of Leukemias in Childhood (GBTLI) 99 and on the Recife Protocol for T-cell Acute Lymphoblastic Leukemia 2011 (RELLA T LLA 2011). Data were obtained by searching physical or electronic medical records. Results: In the selected period, no patients younger than two years old were observed, with 37 (51.4%) patients between 2 and 9 years old, 27 (37.5%) patients between 10 and 15 years old, and 8 (11%) over 15 years old. Based on treatment protocols, 18% of patients were classified as intermediate risk and 82% as high risk The 5-year OS and progression-free survival (PFS) rates were 48% and 43.6%. Age greater than 15 years and the presence of enlarged lymph nodes were independent variables for OS analyses. DRM assessment by CFM was performed only from 2014 onwards, with no linearity. A total of 52 patients were evaluated at the following times: mid-induction (D15-19), end of induction (D29-33) or consolidation of induction (D64-78), with 35, 44 and 24 patients being analyzed, respectively. However, only 16 patients were evaluated in the 3 moments, which made statistical analysis difficult. In the evaluation of the deaths, it was observed that 26.7% of the patients died in remission, among which the main cause of death was infection (91.7%), while 53.3% died with active disease and in 20% it was not possible to assess disease status. The absence of enlarged lymph nodes at diagnosis was identified as a good prognostic factor with an impact on PFS. In the correlations of outcomes with immunophenotypic markers, positivity for CD11b and HLA-DR was associated with a lower rate of OS, on the other hand, the presence of CD8 was associated with a better OS. Conclusions: The evaluations of survival and causes of death in the evaluated population indicate the need for improvements in treatment, especially in clinical support of patients treated in Bahia, which suggests a Brazilian need to be investigated. Immunophenotypic findings with positive (CD8) and negative (CD11b and HLADR) impacts on overall survival were identified. Such findings should be investigated in prospective and multicenter studies, as this is an accessible test in our environment, with possible benefits for the risk classification of T-ALL
