Em 31 de dezembro de 2019, o mundo se deparou com um novo vírus que se espalhou a partir da região Wuhan, província de Hubei, na República Popular da China. Tratava-se de uma cepa de coronavírus que até o momento não tinha sido identificada em humanos. A Organização Mundial de Saúde classificou a situação como emergência internacional e, em 11 de março de 2020, o vírus passou a ser considerado uma doença pandêmica. Covid-19 pertence à família dos coronavírus. Os sintomas da infecção podem evoluir de tosse, febre e congestão nasal à pneumonia grave; alguns indivíduos infectados podem permanecer assintomáticos e contribuir com a propagação do vírus em indivíduos com comorbidades que são mais susceptíveis às manifestações. O presente estudo teve como objetivo analisar a taxa de incidência e de mortalidade por Covid-19 em população da Sub-Bacia Hidrográfica do Canal do Cunha, composta por 36 bairros da cidade do Rio de Janeiro, no período de 12 meses, de 13 de março de 2020 a 05 de março de 2021. O território de estudo é uma região do município do Rio de Janeiro com alta concentração de aglomerados subnormais. O município é divido em quatro grandes compartimentos hidrográficos: Sepetiba, Jacarepaguá, Zona Sul e Guanabara, sendo formado cada um deles por sub-bacia hidrográficas, a Sub-Bacia do Canal do Cunha (SBCC), é uma das sub-bacias da Guanabara, e é predominantemente urbana. Os 29 bairros totalmente inseridos na Sub-bacia são predominantemente urbanos. Dados de óbitos e incidência foram obtidos no site do Painel Rio Covid-19 da Secretaria Municipal de Saúde do Município do Rio de Janeiro. Foram consideradas como variáveis dependentes as taxas de incidência acumulada, mortalidade acumulada e letalidade acumulada. Entre as variáveis contextuais foram o bairro de moradia; proporção de aglomerados subnormais (ASN) por bairros, faixas etárias; razão de renda e índice de saneamento básico. As variáveis foram analisadas de forma descritiva, por estatística bivariada de correlação de Spearman e por estatística espacial. A estatística espacial foi realizada através de Sistemas de Informação Geográfica, utilizando programas livres, QGIS e Terra View. No período de estudo a taxa de incidência nos bairros da SBCC foi de 621,5/10.000 habitantes. Sendo identificada maior taxa de mortalidade (24,7%) no Complexo do Alemão. Obteve-se correlação negativa entre mortalidade e a proporção de habitantes nos bairros com ASN (rho = -0,433; p = 0,023). Na estatística espacial houve correlação inversa para incidência da Covid-19 (índice Moran, -0,155863; p=0,02). O estudo permitiu concluir que a incidência e mortalidade nas áreas de ASN são significativamente relacionadas com a estruturas sociodemográficas, com a demanda de política orientada para territórios vulneráveis. O uso de dados epidemiológicos contribuiu para monitorar a pandemia de Covid-19 e compreender como foram as estratégias adotadas na redução dos casos e óbitos em áreas socioeconomicamente vulneráveis.
Abstract
On December 31st 2019, a new virus appeared to the world, spreading from the Wuhan region, Hubei province, at the Popular Republic of China. It was a new strain of the coronavirus, yet unidentified on humans. The situation was classified by the World Health Organization as a new international emergency, and on March 11th 2020, the virus began to be treated as a pandemic disease, Covid-19, integrating the family of the coronaviruses. Symptoms, which include cough, fever and nasal constipation, can evolve to severe pneumonia. Some infected subjects can even remain asymptomatic, and contribute to the propagation of the virus on others that are more susceptible to the symptoms, such as individuals with comorbidities. This study has the objective of analyzing the Covid-19 incidence and mortality rates on the population of the Canal do Cunha sub-watershed region, which includes 36 districts, on the period starting on March 13th 2020, until March 5th 2021. The aforementioned territory is a region on Rio de Janeiro City with a high concentration of subnormal agglomerates. The city is divided in four big hydrographic compartments: Sepetiba, Jacarepaguá, Zona Sul and Guanabara, each of which formed by a group of sub-watersheds. The Canal do Cunha Sub-Watershed, is a predominantly urban territory, and the same is true on 29 of its districts. The incidence and obits data was obtained on the Painel Rio Covid-19 website, managed by the Rio de Janeiro City Health Secretary. The dependent variables were the accumulated incidence, accumulated mortality and accumulated letality rates. The context variables were the district of residence, the district proportion of subnormal agglomerates, the age, the average income and the basic sanitation index. All of the variables were analyzed on a descriptive manner, using the Spearman bivariate correlation. It was also made a spatial statistic using the Geographic Information System. Incidence rate on the Sub-Watershed districts was of 621.5/10,000 habitants, with a higher 24.7% mortality rate on the Complexo do Alemão district. It was obtained a negative correlation between the mortality and habitants rate on the districts with subnormal agglomerates (rho=-0,433;p=0,023). There also was an inverse correlation on the spatial statistic for the Covid-19 incidence (Moran index, -0,155863; p=0,02). The study helped to conclude that the incidence and mortality rates on the subnormal agglomerate areas are significantly related to the sociodemographic structures. The epidemiologic data contributed to monitor the Covid-19 pandemic and understand how well the adopted strategies were in reducing the cases and obits on socioeconomically vulnerable areas.
