Impurezas de N-nitrosaminas (NA), de conhecido potencial carcinogênico, têm sido motivo de preocupação para a indústria farmacêutica e agências reguladoras em vários países nos últimos anos. NA foram inesperadamente identificadas em medicamentos da classe dos antagonistas de receptores da angiotensina II, as sartanas, agentes anti-hipertensivos mais prescritos. A FDA recolheu produtos farmacêuticos contaminados com NA potencialmente cancerígeno produzidos por várias empresas farmacêuticas e estabeleceu limites de ingestão aceitáveis. No Brasil, a Anvisa recolheu centenas de medicamentos da classe das sartanas e suspendeu a fabricação, importação, distribuição, comercialização e uso dos insumos farmacêuticos ativos com suspeita de contaminação. Métodos normatizados por diversas farmacopeias podem não incluir a análise de possíveis contaminantes que podem surgir no processo produtivo do fármaco, desse modo, impurezas inesperadas podem ser facilmente ignoradas. Um método analítico de quantificação dessas impurezas é necessário para avaliar a qualidade dos medicamentos comercializados no Brasil, para possibilitar a adoção de medidas sanitárias. Métodos de quantificação de seis NA (NDMA, NDEA, NDIPA, NDBA, NEIPA e NMBA) por CLAE-EM/EM em medicamentos de Candesartana e Telmisartana foram desenvolvidos em nosso grupo de trabalho. O espectrômetro de massa com interface APCI em modo de ionização positiva foi empregado em modo de monitoramento de reações múltiplas. Metanol e água foram usados para extração de NA dos comprimidos de sartanas, etapas de agitação por ultrassom e centrifugação foram empregados no método. Os métodos de análise em Candesartana e Telmisartana foram validados de acordo com a RDC Nº 166/2017, curvas analíticas com boa linearidade foram obtidas e as recuperações variaram de 93,4 a 132,0 % em candesartana e 62,3 a 106,2 % em amostras de telmisartana. Os limites de quantificação dos métodos (312,5 µg kg-1 para candesartana e 125 µg kg-1 para telmisartana) atenderam os requisitos de sensibilidade para os limites estabelecidos pelo Guia 50/2021 da Anvisa. Os métodos analíticos foram aplicados com confiabilidade na determinação de impurezas de NA como parte de um programa de monitoramento em parceria com a Anvisa em 6 amostras de candesartana e 6 amostras de telmisartana. Com base nos critérios de limite de aceitação de NA em medicamentos, todas as amostras atenderam aos níveis aceitáveis de N-nitrosaminas.
