As Pessoas que Vivem com HIV (PVHIV) convivem com a ocorrência de discriminação historicamente , t ais episódios têm sido associados a piores resultados de saúde física e mental. O objetivo do estudo foi investigar a associação da discriminação percebida por PVHIV com desfechos de saúde, comparando tais experiências com população não acometida pela infe cção. Trata se de um a pesquisa epidemiológic a que utilizou grupos populacionais distintos: uma amostra de 649 PVHIV acompanhad a s pelo Instituto Nacional de Infectologia Evandro Chagas (INI) e participantes do Estudo Longitudinal de Saúde do Adulto (ELSA Br asil) sem HIV , composto por 15.105 servidores púbicos. Foram realizadas medidas descritivas gerais, regressões logísticas simples múltiplas , comparaç ões entre os grupos por meio do cálculo de escores de propensão e análise de sobrevivência, todas realizad as em ambiente R. O estudo obteve aprovação em comitê de ética em pesquisa com o número de parecer 5.038.275. Verificou se maior chance de se perceber discriminado no grupo de PVHIV quando comparado ao grupo ELSA Brasil (OR=1,10; IC 95% 1,08 1,12). Observ ou se que 48,6% das PVHIV relataram ter sofrido algum tipo de discriminação, com destaque para o domínio locais públicos (22,3%). Dentre os infectados, as variáveis sexo, idade e valor de células CD4 apresentaram associação com a percepção de discriminaçã o, de forma que ser do sexo masculino aumenta va a chance de ter sido discriminado (OR = 1,79; IC95% = 1,40 2,15). Também foi observado que a chance de ter sido discriminado diminu ía 2% a cada ano (OR = 0,98; IC95% = 0,96 0,99). Dentre as razões que os participantes referiram ter passado pelo ato discriminatório, houve destaque para fatores econômicos (30,2%), aparência física (25,80%), ter doença ou deficiência (23,25%) e orientação sexual (22,9%). Na investiga ção d o efeito da discriminação percebida em desfechos de saúde e progressão da doença nas PVHIV, com tempo médio de acompanhamento de 6,9 anos (intervalo interquartil [IQR] 6,8 7,8), houve a ocorrência de 38 óbitos (5,8%), de 41 0 (63,4%) de indivíduos com falha viral ao longo do tempo e de 11,14% de manifestações relacionadas ao HIV (n=72). A discriminação foi apresentada como fator de risco para ocorrência de falha viral e óbito , porém sem significância estatística. Para fins d e redução da discriminação relacio nada às PVHIV , é indispensável que as políticas públicas voltadas ao enfrentamento sejam fortalecidas e implementadas em todos os níveis de atenção a saúde e setores da sociedade.
Abstract
People Living with HIV (PLHIV) have historically lived with the occurrence of discrimination, such episodes have been associated with worse physical and mental health outcomes. The objective of the study was to investigate the association of perceived disc rimination by PLHIV with health outcomes, comparing such experiences with a population not affected by the infection. This is epidemiological research that used different population groups: a sample of 649 PLHIV monitored by the Instituto Nacional de Infec tologia Evandro Chagas (INI) and participants of the Longitudinal Study of Adult Health (ELSA Brasil) without HIV, composed of 15,105 civil servants pubic. General descriptive measurements, simple and multiple logistic regressions, comparisons between grou ps by calculating propensity scores and survival analysis were carried out, all carried out in an R environment. The study was approved by the research ethics committee with the opinion number 5,038,275. There was a greater chance of perceiving discriminat ion in the PLHIV group when compared to the ELSA Brasil group (OR=1.10; 95% CI 1.08 1.12). It was observed that 48.6% of PLHIV reported having suffered some type of discrimination, with emphasis on public places (22.3%). Emphasis on public places (22.3%). Among those infected, the variables sex, age and CD4 cell count were associated with the perception of discrimination, so that being male increased the chance of having been discriminated against (OR = 1.79; CI95% = 1.40 2.15). It was also observed that the chance of having been discrimina ted against decreased by 2% each year (OR = 0.98; 95%CI = 0.96 0.99). Among the reasons that the participants reported having gone through the discriminatory act, there was emphasis on economic factors (30.2%), physical appearance (25.80%), having an illne ss or disability (23.25%) and sexual orientation (22. 9%). In the investigation of the effect of perceived discrimination on health outcomes and disease progression in PLHIV, with a mean follow up time of 6.9 years (interquartile range [IQR] 6.8 7.8), ther e were 38 deaths (5.8%), 410 (63.4%) of individuals with viral failure over time, and 11.14% of HIV related manifestations (n=72). Discrimination was presented as a risk factor for the occurrence of viral failure and death, but without statistical signific ance. In order to reduce discrimination related to PLHIV, it is essential that public policies aimed at coping with it be strengthened and implemented at all levels of health care and sectors of society.
