Extrato medicinal de Cannabis sativa apresenta grande potencial terapêutico e tem sido utilizado mundialmente no auxílio do tratamento de Parkinson, Alzheimer, esquizofrenia, dor neuropática e epilepsia. No Brasil, esse produto é bastante utilizado, principalmente, para controlar crises convulsivas em portadores de epilepsia refratária. A análise microbiológica de extratos de C. sativa é essencial para evitar danos à saúde de usuários, principalmente pacientes imunocomprometidos. O objetivo do presente estudo foi avaliar a identidade, a susceptibilidade aos antimicrobianos e as relações clonais de isolados bacterianos de Escherichia coli (n=60) e Staphylococcus aureus (n=20) obtidos de extratos artesanais de C. sativa (extração oleosa) entre 2017-2018. Para isso, a confirmação da identidade dos isolados foi realizada pela PCR de genes espécie-específicos; a susceptibilidade aos antimicrobianos pela técnica de disco-difusão; as relações clonais pela ERIC-PCR; a determinação da patogenicidade dos isolados de E. coli pela atividade hemolítica e pela pesquisa de genes de virulência de EPEC e EHEC. Os isolados de E. coli e S. aureus foram confirmados pela identificação molecular. Isolados de E. coli revelaram resistência a quatro antimicrobianos: cefuroxima (51,7%); ciprofloxacino (10%); tigeciclina (8,3%) e aztreonam (6,7%), já os isolados de S. aureus apresentaram resistência à benzilpenicilina (40%). Foi revelado um alto polimorfismo genético entre os isolados de E. coli e S. aureus pela ERIC-PCR. Esses isolados não apresentaram atividade hemolítica e nem genes marcadores de virulência. Foi realizada também a pesquisa de contaminação microbiológica e da composição da comunidade microbiana pela metagenômica em 30 amostras de extrato medicinal de C. sativa obtidos por extração alcoólica, entre 2019-2020. Das 30 amostras, sete apresentaram contaminação microbiana e foram identificados fenotipicamente como: Enterobacter cloacae (n=2), Bacillus circulans (n=1), Staphylococcus epidermidis (n=3) e Aspergillus flavus (n=3). A análise metagenômica da comunidade microbiana dos extratos de C. sativa apresentou alta diversidade de organismos do domínio Bacteria (79,7 a 99,4%) com predominância dos filos Proteobacteria, Firmicutes, Bacteroidota e Actinobacteriota; e do domínio Archaea (0,6 a 20,3%). Nossos resultados mostraram uma queda significativa de bactérias recuperadas de extratos obtidos pela extração alcoólica, o que é relevante para diminuição de riscos à saúde de pacientes e garantia da qualidade, segurança e eficácia do produto
