A Barreira Hematoencefálica (BBB) e o líquido cefalorraquidiano (LCR) são componentes do Sistema Nervoso Central (SNC), responsáveis pelo transporte seletivo de substâncias entre este e o sangue periférico. A neuroinflamação é caracterizada pela ativação contínua da micróglia e outras células da BBB. Identificar o perfil de moléculas solúveis no LCR pode auxiliar na investigação dos eventos neuroinflmatórios. Este estudo tem por objetivo avaliar o perfil de moléculas imunológicas solúveis presentes no LCR de pacientes com diferentes doenças neurológicas. Constituíram população de estudo 147 pacientes com dor de cabeça primária, esclerose múltipla (EM), esclerose lateral amiotrófica (ELA) e infecções parasitárias, bacterianas e virais. A quantificação de sHLA-G foi realizada por ELISA e, das demais 35 moléculas, pelo imunoensaio LUMINEX®. Os dados foram analisados usando os softwares GraphPad Prism 5.0 e SPSS e aplicando testes paramétricos e não paramétricos considerando P<0,05. Entre os pacientes recrutados, 56% eram mulheres e a média de idade foi de 40 anos. O aumento da celularidade no LCR foi observado na EM e na doença infecciosa, enquanto que o aumento dos níveis de proteína foi encontrado apenas na doença infecciosa. As enxaquecas apresentaram aumento de sHLA-G proporcional a gravidade da doença, além de aumento dos níveis de IL-8, MIG, IP-10 e IL-9 na forma mais grave da doença, sugerindo correlações reciprocas das moléculas imunes para controle da inflamação. Em EM foi marcante a presença de moléculas inflamatórias ligadas a migração de células T, enquanto que na ELA houve uma redução dessas mesmas moléculas. Em ambas, observou-se uma diminuição da molécula MCP-1, associada a gravidade da doença. Nas infecções bacterianas e parasitárias, as quimiocinas pareceram exercer um papel central na reposta imune no SNC. Na infecção viral observou-se uma reposta padrão independentemente do agente viral, apesar do destaque de IP-10 e MCP-1 na infecção por arbovirus .

The Blood Brain Barrier (BBB) and the cerebrospinal fluid (CSF) are components of the Central Nervous System (CNS), responsible for the selective transport of substances between the CNS and the peripheral blood. Neuroinflammation is characterized by continuous activation of microglia and other cells in the BBB. Identifying the profile of soluble molecules in the CSF can help in the investigation of neuroinflammatory events. This study aims to evaluate the profile of soluble immune molecules present in the CSF of patients with different neurological diseases. The study population consisted of 147 patients with primary headache, multiple sclerosis (MS), amyotrophic lateral sclerosis (ALS) and parasitic, bacterial and viral infections. The quantification of sHLA-G was performed by ELISA and, of the other 35 molecules, by the LUMINEX® immunoassay. Data were analyzed using GraphPad Prism 5.0 and SPSS software and applying parametric and non-parametric tests considering P<0.05. Among the recruited patients, 56% were women and the mean age was 40 years. Increased CSF cellularity was observed in MS and infectious disease, whereas increased protein levels were found only in infectious disease. Migraines showed an increase in sHLA-G proportional to the severity of the disease, in addition to increased levels of IL-8, MIG, IP-10 and IL-9 in the most severe form of the disease, suggesting reciprocal correlations of immune molecules for controlling inflammation. In MS, the presence of inflammatory molecules linked to T cell migration was marked, while in ALS there was a reduction of these same molecules. In both, a decrease in the MCP 1 molecule was observed, associated with the severity of the disease. In bacterial and parasitic infections, chemokines seem to play a central role in the immune response in the CNS. In viral infection, a standard response was observed regardless of the viral agent, although IP-10 and MCP-1 standed out in arbovirus infection .
