Desde a expansão da atenção primária à saúde (APS) no Brasil a partir da criação do Programa Saúde da Família, é notável a contradição entre a crescente demanda por profissionais médicos com formação especializada para atuar no SUS nesse nível de atenção e a escassez de vagas em programas de residência da especialidade. A presente dissertação tem por objetivo avaliar o processo de implementação do programa de residência médica em medicina de família e comunidade (MFC) da Secretaria de Saúde do Recife (Sesau-Recife), entre 2013 e 2019. Consiste em um estudo de caso, de abordagem qualitativa, de caráter descritivo e analítico. Utilizamos, para a coleta de dados, dois métodos de pesquisa: a análise documental relacionada a documentos públicos de interesse; e a realização de entrevistas semiestruturadas com informantes-chave escolhidos entre os gestores da secretaria de saúde da cidade do Recife (PE), entre trabalhadores de saúde diretamente ligados ao PRMFC, além de médicos egressos do programa. A análise das entrevistas foi realizada por meio da análise de conteúdo. Entre os resultados da análise documental, apresentamos uma caracterização sociopolítica da cidade do Recife e reunimos a legislação, decretos, portarias e resoluções de interesse para a implementação de PRMFCs. Entre os resultados da análise de conteúdo da transcrição das entrevistas realizadas com informantes-chave, apresentamos categorias iniciais, intermediárias e final. Realizamos a avaliação da implementação do programa à luz dos critérios de economicidade, eficiência administrativa, eficácia, envolvimento dos preceptores de MFC, formação pedagógica, avaliação de aprendizagem, dos estágios e preceptores, contribuição ao desenvolvimento do sistema local de saúde. A avaliação do processo de implementação foi predominantemente positiva. O estudo apresenta algumas limitações inerentes às opções metodológicas escolhidas, somadas ao desafio de fazer pesquisa em um contexto de pandemia da Covid-19, com menor possibilidade de acesso a documentos físicos.
Abstract
Since the expansion of Primary Health Care (PHC) in Brazil with the creation of the Family Health Program, there is a contradiction between the growing demand for medical professionals with specialized training to work in the SUS at this level of care and the scarcity of vacancies in specialty residency programs. This Master Work aims to evaluate the implementation process of the medical residency program in family and community medicine (MFC) of the Health Secretariat of Recife (Sesau-Recife), between 2013 and 2019. It consists of a case study, from a qualitative, descriptive and analytical approach. For data collection, we used two research methods: document analysis related to public documents of interest; and semi-structured interviews with key informants chosen among the managers of the health secretariat of Recife/PE, among health workers directly linked to the PRMFC, and physicians who graduated from the program. The analysis of the interviews was carried out through content analysis. Among the results of the document analysis, we present a sociopolitical characterization of the city of Recife and gather legislation, decrees, ordinances and resolutions of interest for the implementation of PRMFCs. As the results of the content analysis of the transcript of interviews conducted with key informants, we present initial, intermediate and final categories. We carried out the evaluation of the program's implementation in light of the criteria of economy, administrative efficiency, effectiveness, involvement of MFC preceptors, pedagogical training, assessment of learning, internships and preceptors, and contribution to the development of the local health system. The evaluation of the implementation process was predominantly positive. The study has some limitations inherent to the chosen methodological options. Furthermore, there is the challenge of conducting research in a pandemic context, with limited possibility of accessing physical documents.
