INTRODUÇÃO: Desde o aparecimento da COVID-19, numerosos estudos observacionais buscam o melhor entendimento e predição da síndrome respiratória aguda grave e da grave resposta inflamatória sistêmica por meio da associação com comorbidades, variáveis hematológicas, inflamatórias, bioquímicas e da coagulação, além da cinética da carga viral. Entretanto, permanecem lacunas no estudo clinicopatológico da COVID-19 fatal. Por isso, este estudo tem como objetivo apresentar uma análise dos fatores clínicos-demográficos e laboratoriais junto às lesões histopatológicas. MÉTODOS: Trata-se de estudo unicêntrico, observacional e transversal de 50 casos de COVID-19 que evoluíram a óbito e foram submetidos a autópsia minimamente invasiva. Dados clínicos-demográficos, laboratoriais e radiológicos foram acessados em revisões dos prontuários clínicos. RESULTADOS: A idade média foi de 65 anos e as comorbidades mais prevalentes foram hipertensão arterial sistêmica, diabetes mellitus e obesidade. As principais alterações laboratoriais da admissão foram linfopenia, níveis aumentados de PCR, LDH, dímero-D, creatinina, ureia e razão PaO₂/Fi0₂ abaixo de 300 mmHg. As principais alterações laboratoriais observadas no dia do óbito foram leucocitose, neutrofilia, PCR elevada, insuficiência renal, razão PaO₂/Fi0₂ abaixo de 300 mmHg. A análise histopatológica mostrou acometimento inflamatório de múltiplos órgãos e os achados principais foram: dano alveolar difuso - DAD (64,4%), miocardite (36,2%), hepatite (71,4%) e nefrite intersticial (21,4%). O acometimento inflamatório de múltiplos órgãos mostrou correlação estatisticamente significativa com síndrome metabólica e DM (p 0,009 e p 0,005, respectivamente), 90% dos que apresentaram nefrite eram diabéticos (p 0,012); houve correlação entre miocardite e síndrome metabólica (p 0,001) e entre miocardite e obesidade (p 0,005). Entre os escores clínicos, o SOFA no quinto dia de internação apresentou associações com dano alveolar difuso – DAD - e resposta inflamatória em múltiplos órgãos - RIMO. Na análise discriminante, foram elaborados modelos preditivos com 71% e 72% de acurácia para DAD e RIMO, respectivamente. CONCLUSÃO: A presença de comorbidades e escores clínicos até o quinto dia de internação combinados com indicadores de lesão inflamatória podem ser mais sensíveis para predizer a COVID-19 fatal.

INTRODUCTION: Since the emergence of COVID-19, there have been several studies seeking to better understand and predict severe acute respiratory syndrome and severe systemic inflammatory response by association with comorbidities, hematologic, inflammatory, biochemical, and coagulation variables, and viral load kinetics. However, gaps remain in the clinicopathological study of these prognostic factors in fatal COVID-19. Therefore, this study aims to present a relationship of clinical-demographic and laboratory prognostic factors with histopathological lesions. METHODS: This is a single-center, observational, cross-sectional study of 50 cases who underwent minimally invasive autopsy. Clinical-demographic, laboratory, and radiological data were accessed in reviews of clinical records. RESULTS: The mean age was 65 years, and the most prevalent comorbidities were hypertension, diabetes mellitus, and obesity. The main laboratory changes on admission were lymphopenia, increased levels of C - reactive protein, lactate dehydrogenase (LDH), D-dimer, creatinine, urea and PaO₂/Fi0₂ ratio below 300 mmHg. The main laboratory alterations observed on the day of death were leukocytosis, neutrophilia, increased levels of C - reactive protein, renal failure and PaO₂/Fi0₂ ratio below 300 mmHg. Histopathological analysis showed multiple organ inflammatory involvement and the main findings were diffuse alveolar damage - DAD (64.4%), myocarditis (36.2%), hepatitis (71.4%) and interstitial nephritis (21.4%). Multiple organ inflammatory showed statistically significant correlation with metabolic syndrome and DM (p 0.009 and p 0.005, respectively), 90% of those who had nephritis were diabetic (p 0.012); there was correlation between myocarditis and metabolic syndrome (p 0.001), myocarditis and obesity (p 0.005). Among the clinical scores, SOFA on the fifth day of hospitalization showed associations with DAD and multiple organ inflammatory. In the discriminant analysis, predictive models with 71% and 72% accuracy were developed for DAD and multiple organ inflammatory, respectively. CONCLUSION: The presence of comorbidities and clinical scores by the fifth day of hospitalization combined with indicators of multiple inflammatory injury may be more sensitive for predicting fatal COVID-19.
