Acolhimento e vínculo são conceitos complexos e multidimensionais. No
presente estudo, o acolhimento foi operacionalizado nas dimensões de acesso, atenção
ao doente, humanização e autonomia. O vínculo foi analisado nas dimensões de
confiança, responsabilidade e longitudinalidade. Acolhimento e vínculo são ferramentas
que auxiliam na reorganização do processo de trabalho em saúde, influindo na
qualidade do cuidado e proporcionando uma atenção mais efetiva e humanizada.
Esse trabalho teve como objetivo efetuar uma análise sobre o acolhimento e
vínculo ao portador de diabetes em um Centro de Saúde Escola (CSE) no qual atuam
oito equipes de saúde da família e que também atende através do modelo tradicional
(unidade com modelo assistencial misto). Os objetivos específicos da pesquisa foram:
avaliar o acolhimento e a criação de vínculo entre unidade de saúde/profissionais da
ESF e usuários portadores de diabetes, examinar aspectos de acolhimento e vínculo
relacionado à estrutura, englobando instalações, recursos materiais, humanos e
características organizacionais e comparar o acolhimento e vínculo proporcionado entre
as ESF completas e incompletas.
O diabetes mellitus foi escolhido como traçador para avaliar o acolhimento e
vínculo no PSF em razão de sua elevada prevalência, com influência direta sobre a
qualidade de vida dos portadores e sobre os custos para o Sistema Único de Saúde.
O delineamento da pesquisa foi transversal e os instrumentos utilizados para a
coleta de dados foram: roteiro para observação participante do processo de atenção nas
diversas etapas do atendimento (agendamento de consultas, recepção e triagem, préconsulta, consulta e reunião de grupo de diabéticos) e aplicação de questionários em
amostra de portadores de diabetes II selecionada do banco de dados do CSE a partir de
uma amostra aleatória simples com partilha proporcional entre usuários assistidos por
ESF completas e ESF não estruturadas. O inquérito contemplou 53 usuários que
responderam ao questionário em suas residências.
Os dados foram analisados comparando-se respostas dos usuários das ESF
completas (5) e de ESF incompletas (3). Os usuários fizeram uma avaliação positiva da
assistência recebida no CSE. As dimensões do acolhimento foram bem avaliadas com
destaque para a humanização que foi avaliada de forma muito positiva por quase todos
os usuários sendo o acesso a dimensão que os usuários tiveram maior dificuldade em
razão do CSE ainda ser utilizado também no modelo tradicional de queixa consulta. As
dimensões do vínculo também apresentaram resultados positivos sendo melhores para
as ESF estruturadas principalmente na dimensão de responsabilidade com melhor
orientação para medidas preventivas. A confiança nos médicos do PSF e a intenção de
continuar o tratamento no CSE foram fatores positivos constatados na pesquisa.
Ainda que os resultados do inquérito não possam ser generalizados, o estudo
sugere que o modelo assistencial do PSF pode proporcionar o estabelecimento de
acolhimento e vínculos aos usuários.
Abstract
Receptiveness and trust relationship in health care are complex and
multidimensional concepts. In the present study, receptiveness was operationalised in
the dimensions: access, attention to the ill, humanization and autonomy. Trust
relationship in health care was analyzed in the trust, responsibility and longitudinal
dimensions. Receptiveness and trust relationship in health care are tools that help in the
re-organization of the work process in health, influencing in the quality of care and
providing a more effective and humanized attention.
This work had as objective makes a analysis about receptiveness and trust
relationship in health care to the patient with diabetes in a School Health Center (CSE)
in work eight family health teams and that also care through the traditional model
(mixed assistance model). The specific objectives of the research were: to evaluate the
receptiveness and trust relationship in health care creation among health unit/ ESF
professionals and patient with diabetes, to examine receptiveness and trust relationship
in health care aspects related to the structure, covering installations, material and human
resources and organization characteristics and to compare the receptiveness and trust
relationship in health care provided, between the complete and incomplete ESF.
Diabetes mellitus was chosen as tracer to evaluate receptiveness and trust
relationship in health care in PSF, due to its high prevalence, with direct influence over
the quality of life of the people with diabetes and the costs for the Unified Health
System.
The research design was transversal and the instruments used for the data
collection were: schedule for observation of health care in the various stages of
treatment (appointments, reception and selection, pre-consultation, consultation and
meeting with group of people with diabetes ) and application of the questionnaires in
samples of people with diabetes II selected from the CSE data bank from a simple
random sample with proportional split among users assisted by complete and non
structured ESF. The questioning reached 53 users, who answered the questioner in their
homes.
Data was analyzed by comparing answers from users of the complete (5) and
incomplete (3) ESF. Users made use of a positive evaluation of care received in the
CSE. The receptiveness and trust relationship in health care were well evaluated, having
as highlight the humanization which was evaluated in a very positive form by almost all
users, being the access to the dimension that the users had more difficulty because the
CSE be still used also in the traditional model of the “visit-complaint”. The dimensions
of the trust relationship in health care also presented positive results, being better for the
structured ESF especially in the dimension of responsibility with better orientation for
the prevention measures. Confidence in the PSF physicians and the intention to continue
treatment in CSE were positive factors identified in the research.
Although the outcomes of the inquiry cannot be widespread, the study suggests
that the model assistencial of PSF can provide the receptiveness and trust relationship to
the users.
