Resumo: A hipertensão arterial e o diabetes constituem-se em importantes fatores de
risco (FR) para as doenças cardiovasculares (DCV). Um dos maiores desafios no
combate à HAS e ao diabetes é a não aderência à terapêutica. Objetivo: Investigar a
prevalência de FR cardiovasculares e a adesão ao tratamento em pacientes cadastrados
no Sistema de Acompanhamento de Hipertensos e Diabéticos (HIPERDIA) em unidade
de referência de Fortaleza, Ceará, 2002 - 2005. Métodos: A dissertação está estruturada
na forma de dois artigos: primeiro artigo: estudo observacional e transversal com 682
pacientes inscritos no HIPERDIA do Centro Integrado de Diabetes e Hipertensão
(CIDH), em Fortaleza-Ceará, entre setembro de 2002/fevereiro de 2003; segundo artigo:
seguimento retrospectivo (24 meses) de uma amostra probabilística com 249 pacientes
inscritos no HIPERDIA do CIDH, em Fortaleza-Ceará, com seguimento entre setembro
de 2002/fevereiro de 2005. A coleta de dados foi efetuada a partir das fichas de
cadastro do HIPERDIA e por intermédio dos prontuários do CIDH. Descreveram-se as
distribuições de prevalências das variáveis dos estudos e realizaram-se as razões de
prevalências para FR potencialmente associados aos desfechos, com IC = 95%.
Resultados: Primeiro artigo: observou-se igual prevalência de sedentarismo em homens
(52,7%) e mulheres (55,4%). O tabagismo (24,3%) foi mais prevalente nos homens,
enquanto que (68,9%) das mulheres tinham a cintura acima do anormal e (31%) eram
obesas. A maioria dos pacientes cadastrados era hipertenso e diabético (55,5%). O IAM
(8,8%) e outros eventos coronarianos (11,0%) foram mais prevalentes entre as
mulheres. Uma associação positiva, com DCV e sedentarismo (RP=1,46), cintura
(RP=1,89) e faixa etária (RP= 1,99) foi obtida, com significância estatística. Segundo
artigo: observou-se que os homens (47, 8%) foram menos aderentes ao tratamento, que
as mulheres (42,2%) e a não aderência foi mais prevalente no 2º ano do seguimento. A
associação entre os FR e da não aderência, não apresentou significância estatística em
ambos os sexos. Conclusão: os pacientes do CIDH cadastrados no HIPERDIA fazem
parte de uma população de elevado risco para a DCV e de elevada prevalência de não
aderência ao tratamento. Faz-se necessário, portanto, que programas de intervenção
multidisciplinar sejam implementados, na tentativa de modificar este quadro.
Abstract
Summary: The hypertension and diabetes consist in important factors of risk (FR) for
cardiovascular disease (DCV). One of the biggest challenges in the combat to the HAS
and diabetes, is not the adherence to the therapeutically. Objective: To investigate the
prevalence of cardiovascular FR and adhesion to the treatment in patients registered in
cadastre in the System of Accompaniment of Hipertensos and Diabéticos (HIPERDIA)
in unit of reference of Fortaleza, Ceará, 2002 - 2005. Methods: The thesis is
structuralized in the form of two articles: first article: observational and transversal
study with 682 enrolled patients in the HIPERDIA of the Integrated Center of Diabetes
and Hypertension (CIDH), in Fortaleza-Ceará, between September of 2002/fevereiro of
2003; second article: retrospective cohort (24 months) with 249 enrolled patients in the
HIPERDIA of the CIDH, in Fortaleza-Ceará, between September of 2002/fevereiro of
2005. The collection of data was effected from the fiches of registers in cadastre of the
HIPERDIA and for intermediary of handbooks of the CIDH. The distributions of
prevalence of the variable of the studies had described and reasons of prevalence for
potentially associated FR to the outcomes, with IC = 95%. Results: First was observed
equal prevalence of physical inactivity in men (52.7%) and women (55.4%). The
smoking (24.3%) had been more prevalent in the men, whereas (68.9%) of the women
had the waist above of the normal and 31% had obesity. The majority of the registered
in cadastre patients was hypertensions and diabetic (55.5%). %). The myocardial
infarction (8.8%) and another disease ischemic (11.0%) had been more prevalent
between the women. A positive association with DCV and physical inactivity (RP=1,
46), waist (RP=1, 89) and age (RP= 1, 99) was found with significance. Second article:
it was observed that the men (47, 8%) had been less adherent than the women (42.2%)
to the treatment and the prevalence of adherence was not bigger in 2º year of the cohort.
The association between the FR and not adherence was not significant in both the
genders. Conclusion: the patients of the CIDH registered in cadastre in the HIPERDIA
are part of a population of raised risk for the DCV and of raised prevalence of not
adherence to the treatment. One becomes necessary, therefore, that intervention
programs to multidiscipline are implemented, in the attempt to modify this situation.
