Este trabalho se caracteriza uma pesquisa avaliativa acerca da efetividade do Programa Mais Médicos (PMM) no enfrentamento de problemas da Atenção Primária à Saúde (APS) no Nordeste. O estudo foi realizado por meio da integração de métodos quantitativos e qualitativos. Foram conduzidas três linhas de investigação, entrelaçadas: avaliar a influência do PMM sobre a ocorrência de internações por condições sensíveis à APS; verificar o acesso e a utilização de serviços de saúde na APS, antes e a partir do programa; e identificar sua repercussão na integralidade. Os resultados são apresentados em quatro artigos, com opções metodológicas delineadas em função do objeto de estudo retratado em cada manuscrito. Os principais resultados evidenciam que houve queda de 35% na média de internações por Diarreia e Gastroenterite, uma condição sensível com histórica ocorrência no Nordeste. A força de trabalho médica na APS teve ampliação significativa, com aumento da cobertura da Estratégia de Saúde da Família (ESF) na Região. Entre 2013 e 2015, o estudo revela aumento de 19,2% no quantitativo de consultas médicas realizadas na ESF. No escopo da integralidade, a percepção de entrevistados sugere avanços na construção de vínculo entre profissionais e usuários, melhor comunicação entre estes atores nas consultas e maior qualidade na realização das práticas do cuidado em saúde. Apesar desses avanços, a capacidade da APS de ordenar o cuidado nas redes de atenção à saúde mostrou-se prejudicada, sobretudo, em função das dificuldades em encontrar retaguarda para as demandas geradas, tais como a realização de exames, consultas e procedimentos nos serviços da atenção especializada. Embora o incremento de profissionais proporcionado pelo programa, pode-se inferir que persiste carência de médicos na APS da Região. Em que pese as dificuldades existentes, os resultados apresentados fundamentam a tese de que o PMM se mostrou efetivo nos aspectos avaliados, em cada linha de investigação .
Abstract
This study is an evaluation research about the effectiveness of the More Doctors Program in facing Primary Health Care (PHC) problems in the Northeast. The study was carried out through the integration of quantitative and qualitative methods. Three intertwined lines of investigation were conducted: to evaluate the influence of More Doctors Program on the occurrence of hospitalizations due to PHC sensitive conditions; verify the access and use of health services in PHC, both before and after the program; and to identify its repercussion in integrality. The results are presented in four articles, with methodological options delineated according to the object of study portrayed in each manuscript. The main results show that there was a decrease of 35% in average of hospitalizations for diarrhea and gastroenteritis, a sensitive condition with historical occurrence in the Northeast. The medical workforce in PHC has expanded significantly and increased coverage of the Family Health Strategy (FHS) in the Region. Between 2013 and 2015, the study shows a 19.2% increase in the number of medical consultations performed at the FHS. Regarding integrality, the interviewees' perception suggests advances in building bond between professionals and users, better communication between these subjects in the consultations, and higher quality in the of health care practices. Despite these advances, PHC's capacity to organizer care in health care networks has been impaired, mainly due to the difficulties to perform exams, consultations and procedures in the health services of the specialized attention. Although the increase of professionals provided by the program, it can be inferred that there is still a shortage of doctors in the PHC of the Region. Even in the face of difficulties, the presented results support the thesis that the More Doctors Program had effectiveness in the evaluated aspects, in each line of investigation .
