Abstract in Portuguese
Analisamos nesta dissertação os processos na gestão pública da Secretaria Municipal de Saúde do Rio de Janeiro associados à continuidade e/ou descontinuidade na implementação das políticas de Atenção Básica. Constata-se que as políticas públicas são permeadas por ações fragmentadas, conflitos entre gestores, rotatividade de profissionais, personificação de políticas e planejamento de curtos prazos baseados em ciclos eleitorais. Neste sentido, partimos de uma determinada concepção de Estado e do contexto de reforma do Estado, juntamente com a cultura da Administração Pública brasileira, para compreender os processos multideterminados em torno desta questão. A expansão e a retração dos serviços da Atenção Primária à Saúde no Rio de Janeiro expressam esta lógica e a disputa de interesses em torno da política de saúde, tornando-se por isso mesmo conjunturas ricas para desvelar questões estruturais que afetam a implementação de políticas. Além disso, as políticas de Atenção Básica têm um papel fundamental na concretização do SUS. Acreditamos que o diálogo com os servidores públicos com experiência em cargos de confiança na gestão do município do Rio de Janeiro oportunizaria o reconhecimento e análise de tais processos. São trabalhadores que exercem funções comprometidas com o projeto de governo eleito, ao mesmo tempo, que no papel de estatutários tem garantido o direito de estabilidade, sendo uma possível blindagem para atos de resistências e inflexões dos caminhos das políticas públicas. O desenvolvimento deste trabalho foi baseado em entrevistas semi-estruturadas, na revisão da literatura pertinente ao tema e análise dos documentos públicos que tratam das questões sobre gestão pública, políticas de Atenção Básica e Estado. O estudo teve um caráter qualitativo visto que essa opção metodológica propicia a imersão nas relações com os sujeitos da pesquisa e a apreensão suas concepções a respeito do objeto de estudo. Por fim, buscamos na pesquisa respostas que nos subsidiem na luta pela continuidade das políticas públicas, especificamente da saúde.
Abstract
In this dissertation, we analyze the processes in the public management of the Municipal Health Department of Rio de Janeiro associated with continuity and/or discontinuity in the implementation of Primary Care policies. It appears that public policies are permeated by fragmented actions, conflicts between managers, professional turnover, personification of policies and short-term planning based on electoral cycles. In this sense, we start from a certain conception of the State and the context of State reform, together with the culture of Brazilian Public Administration, to understand the multidetermined processes around this issue. The expansion and contraction of Primary Health Care services in Rio de Janeiro express this logic and the dispute of interests around health policy, thus becoming rich conjunctures to unveil structural issues that affect the implementation of policies. In addition, Primary Care policies play a key role in the implementation of the SUS. We believe that dialogue with public servants with experience in positions of trust in the management of the municipality of Rio de Janeiro would provide opportunities for the recognition and analysis of such processes. They are workers who perform functions committed to the elected government project, at the same time, in the role of statutory, they have guaranteed the right to stability, being a possible shield for acts of resistance and inflections of the paths of public policies. The development of this work was based on semi-structured interviews, a review of the literature relevant to the topic and an analysis of public documents that deal with issues of public management, primary care policies and the State. The study had a qualitative character since this methodological option provides an immersion in the relationships with the research subjects and the apprehension of their conceptions regarding the object of study. Finally, we seek in the research answers that support us in the struggle for the continuity of public policies, specifically health.
