A leptospirose, uma das zoonoses mais difundidas no mundo, é um importante
problema de saúde pública, sobretudo em países de clima tropical onde é responsável
por surtos da doença logo após o período de maior precipitação pluvial. Em Salvador, a
leptospirose tem sido a causa de epidemias cíclicas associadas às chuvas no ambiente
urbano. As condições climáticas e a crescente pobreza urbana têm contribuído para o
aparecimento de grandes surtos associados à alta letalidade a cada ano, durante o
mesmo período sazonal e acometendo os mesmos grupos de risco dentro das
comunidades das favelas urbanas. O objetivo dessa dissertação é descrever a
distribuição espacial dos casos de leptospirose, buscando identificar áreas de risco para
ocorrência da doença, na cidade de Salvador, no período de 1996 a 2006, segundo
fatores sócio-econômicos e ambientais. Como metodologia, utilizou-se a técnica de
análise espacial de Kernel, que consiste na criação de uma razão entre duas
superfícies que permite visualizar a superfície de risco da doença, onde o numerador
compreende os casos e o denominador um Kernel da população dos setores
censitários. Através deste método foi possível delimitar áreas de maior densidade de
casos, e compará-las visualmente com áreas consideradas de risco para ocorrência da
doença, ou seja, onde há presença de favelas, menor proporção de coleta de lixo e
maior proporção de chefes de família com renda média de 1 salário mínimo.
Comparando os períodos de chuva com os períodos de seca, durante os anos do
estudo, verificou-se a presença de padrão espacial semelhante nos dois períodos,
sobreposto às áreas de maior risco para ocorrência da doença.
Abstract
The leptospirosis, one of the most diffused zoonosis in the world, is an important
problem of public health, mainly in tropical countries where it causes several episodes of
the disease after the rainfall period. In Salvador, the leptospirosis has been the cause of
cyclic epidemics associated to the rainfall in the urban environment. The weather
conditions and the increasing urban poverty has been contributed to the new and
massive episodes associated to the high lethality every year, during the same seasonal
period and reaching the same risk groups within communities living in urban slums. The
aim of this study was describe the spatial distribution of the cases of leptospirosis in
order to identify the risk areas where the disease occurs in the city of Salvador from
1996 to 2006, according to social-economic and environmental aspects. As methods, it
was used the Kernel Spatial Analysis which consists on the ration between two planes
that allows to view the plane of the risk of the disease, which the numerator corresponds
to the cases and the denominator corresponds to a Kernel of the population according to
the census tracts. Using this method it was possible to define areas of higher density of
cases and compare them (on the eye basis) to the areas considered as risk areas to the
raise of the disease. In other words, where there are slums, minor proportion of trash
service and higher proportion of families with a monthly income of one brazilian salary in
average. Comparing rainfall periods with non-rain periods, during the years evaluated in
the present study, it was found the presence of a spatial pattern was similar to the two
periods in an overlap of the areas of the higher risk to the raise of the disease.
