Resumen en portugues
O propósito deste estudo é a compreensão do papel exercido pelas transferências de
recursos intergovernamentais do Sistema Único de Saúde no contexto do federalismo fiscal
brasileiro, instaurado após a Constituição Federal de 1988, bem como seu significado para
a implantação de um modelo assistencial, de abrangência federativa.
Seu desenvolvimento está baseado na hipótese de que o fortalecimento da esfera
federal frente aos demais entes da federação favoreceu o estabelecimento de regras e
condicionalidades, pelas quais as transferências intergovernamentais serviram como
instrumentos importantes para a consolidação e disseminação do modelo assistencial
federativo do SUS. Para examinar a validade desta hipótese buscou-se o entendimento do
histórico do federalismo fiscal no Brasil, desde a instauração da República, até os dias
atuais. Abordou-se, com maior ênfase, a transição do regime autoritário para a democracia
e seu contexto de crise fiscal e federativa. Procurou-se caracterizar o desenho da partilha
fiscal brasileira e seus instrumentos redistributivos, destacando-se as transferências
voluntárias que incluem os repasses para o Sistema Único de Saúde.
O resultado da análise revela que a partir de meados da década de 1990, a União
passa a utilizar-se de mecanismos fiscais, frente à condição de fragilidade dos estados,
naquele período, e robustece sua posição frente ao conjunto da federação brasileira. O
fortalecimento de seu papel reflete-se na política setorial da saúde, em que as normas e
mecanismos financeiros por elas instaurados servirão como elementos fundamentais na
consolidação e disseminação do modelo assistencial definido, à época, para o SUS.
Resumen en ingles
The aim of this study is to understand the role of intergovernmental transfers in the field of
the Brazilian National Health System (SUS), created after 1988 Federal Constitution. It is
also intended to explore the meaning of these transfers for the implementation of a
healthcare model under a federative perspective.
Our central hypothesis is that the increasing political power of the central government led
the Union to rule intergovernmental transfers in a way to consoled the federative SUS
healthcare model.
To check this hypothesis we have studied the historical development of the Brazilian fiscal
federalism, since the Proclamation of the Republic until present times. Our focus was on
transitional period from dictatorship to democracy and its fiscal and federative crises. We
have discussed the characteristics of the Brazilian federative fiscal distribution, dedicating
special attention to the “transferências voluntárias” (volunteer transfers) that include
transfers to SUS.
Our analysis shows that during the mid-nineties, the Union explored the states weaknesses
and assumes a stronger position in the Brazilian federation. This change was reflected in
the health sector policy and its rules. In the end, healthcare policy financial mechanisms
were essential for the healthcare model dissemination through SUS.
