A Covid-19 foi identificada pela primeira vez no Brasil em fevereiro de 2020, na cidade de São Paulo. A doença se concentrou primeiramente nos grandes centros, mas se espalhou de forma rápida para o restante do país, atingindo de forma diferencial as populações socioeconomicamente vulneráveis. O grupo ocupacional dos trabalhadores de frigoríficos esteve entre os mais afetados, devido a características da dinâmica e do ambiente de trabalho. Visto a histórica contribuição das análises espaciais e espaço-temporais para o entendimento da dinâmica de doenças infecciosas, o estudo se propôs a estimar, através de modelagem estatística sob a perspectiva Bayesiana, a distribuição espaço-temporal de Síndrome Respiratória Aguda Grave causada por Covid-19 (SRAG-COVID) no estado de São Paulo entre março de 2020 e agosto de 2021. Atentou-se para as especificidades do processo de disseminação pelo território e para a relação dessa distribuição com fatores socioeconômicos, demográficos e a presença de frigoríficos. No período de estudo registrou-se 509.855 casos e 147.070 óbitos por SRAG-COVID. Houve aglomerados iniciais de casos e óbitos na Região Metropolitana de São Paulo e um posterior movimento rumo às regionais do interior do estado. Essas regionais foram as mais afetadas durante o ano de 2021, ano que, até o mês de agosto, havia registrado 57,38% mais casos e 95,67% mais óbitos do que o ano de 2020 completo. Para a modelagem, considerou-se mais adequado o modelo com componentes espacial e temporal estruturados e uma interação espaço-tempo a nível de região de saúde. Os coeficientes das covariáveis foram mantidos fixos no tempo. Municípios maiores e de melhor índice socioeconômico se mostraram mais afetados em todo o período para casos e óbitos, e a presença de frigoríficos se apresentou como um fator associado ao aumento da incidência e mortalidade por SRAG-COVID nos municípios, mesmo após considerar potenciais variáveis confundidoras. O estudo contribuiu para a descrição da dinâmica espaço-temporal da doença no estado e trouxe destaque à manifestação do efeito das precárias condições de trabalho sobre os desfechos da Covid-19 em nível ecológico, trazendo para a discussão a histórica invisibilização desses trabalhadores e a contraditória definição de essencialidade do trabalho que não serve aos interesses da população, mas à manutenção dos patamares de acumulação.
Abstract
Covid-19 was first identified in Brazil in February 2020, in the city of São Paulo. It has first affected larger cities but it rapidly spread to the rest of the country, impacting overall socioeconomically vulnerable populations. The occupational group of slaughterhouse workers has been one of the most affected, due to the job dynamics and the work environment. Given the historical contribution of spatial and spatio-temporal analysis to the understanding of infectious disease dynamics, the present study aimed to estimate, through statistical modeling under the Bayesian perspective, the spatio-temporal distribution of Severe Acute Respiratory Infections caused by Covid-19 (SARI-COVID) in the state of São Paulo between March 2020 and August 2021. We draw attention to the complexities and specificities in the spread of the disease, and to its relation with socio-economic and demographic factors and the presence of slaughterhouses in the municipalities. In the study period, 509,855 SARI-COVID cases and 147,070 deaths were registered. There has been an initial clustering of cases and deaths in the metropolitan region of São Paulo, followed by a movement to the regions in the state countryside. These regions held the highest case and death rates in 2021 (up until August), which have been respectively 57.38% and 95.67% higher than 2020. The larger and more socioeconomically favorable municipalities were most affected throughout the study period, and the presence of slaughterhouses was associated with an 8% increase in both SARI-COVID incidence and mortality, even after adjustment by potential confounders. The study therefore contributed to the characterization of the spatio-temporal dynamics of Covid-19 in São Paulo state, and shed light over the effect of precarious working conditions in Covid-19 outcomes in the ecological level. We bring to the discussion the historical marginalization of slaughterhouse workers and the contradictory definition of work essentiality, which does not favor the population but capital accumulation and the structural maintenance of inequalities.
