A regulação assistencial busca, entre seus objetivos centrais, a garantia do acesso com
equidade, elemento ainda crítico na interface entre APS e atenção especializada no SUS. A
partir de 2012, na atenção primária da cidade do Rio de Janeiro médicos das unidades
passaram a exercer também a função de reguladores do acesso a exames e consultas
ambulatoriais. A singularidade deste desenho descentralizado de regulação (considerando o
cenário nacional) bem como a importância da regulação para a coordenação do cuidado
(função estratégica da APS) compõem a justificativa do presente estudo, que objetivou
explorar tal experiência de regulação ambulatorial, notadamente as suas implicações para o
acesso dos usuários à atenção ambulatorial especializada. Trata-se de pesquisa qualitativa,
realizada a partir de entrevistas (com profissionais e gestores) e análise documental. Os
resultados foram organizados em torno das seguintes dimensões: Trajetórias e nuances do
processo organizativo da regulação assistencial, o funcionamento local da regulação
ambulatorial descentralizada nas unidades de APS e efeitos para o acesso à atenção
especializada. Evidenciou-se que a descentralização da regulação ambulatorial na experiência
estudada contribui para o aumento do protagonismo da APS na regulação e para ganhos
parciais na função de coordenação do cuidado a frágil preparação para o exercício da
regulação, a competição por vagas, a sobrecarga profissional e a falta de equidade do modelo
foram considerados desafios para o exercício da regulação assistencial descentralizada. Tal
modalidade de regulação é incapaz, por si mesma, de resolver problemas que dizem respeito à
oferta de ações e serviços especializados, ao modelo de cuidado na atenção especializada. De
todo modo, trata-se de inovação promissora, ainda que requeira aprimoramentos substanciais.
Resumen en ingles
The care regulation seeks, among its central objectives, to guarantee access with
equity, which is still a critical element in the interface between PHC and specialized care in
the SUS. As of 2012, in the primary care of the city of Rio de Janeiro, physicians from the
units began to also exercise the role of regulators of access to examinations and outpatient
consultations. The uniqueness of this decentralized regulatory design (considering the national
scenario) as well as the importance of regulation for the coordination of care (strategic role of
the PHC) make up the justification for this study, which aimed to explore this experience of
outpatient regulation, notably its implications for users' access to specialized outpatient care.
It is a qualitative research, carried out through interviews (with professionals and managers)
and document analysis. The results were organized around the following dimensions:
Trajectories and nuances of the organizational process of care regulation, the local functioning
of decentralized outpatient regulation in PHC units and effects on access to specialized care. It
was evident that the decentralization of outpatient regulation in the studied experience
contributes to the increased role of PHC in regulation and to partial gains in the care
coordination function, the fragile preparation for the exercise of regulation, competition for
vacancies, professional overload and the lack of equity in the model were considered
challenges for the exercise of decentralized care regulation. This modality of regulation is
incapable, by itself, of solving problems related to the provision of specialized actions and
services, to the model of care in specialized care. In any case, this is a promising innovation,
even though it requires substantial improvements.
