A malária é uma doença negligenciada de grande impacto na saúde pública que acomete regiões de baixa renda e difícil acesso. Seguindo a atual estratégia do governo brasileiro para o combate a essa enfermidade, onde reduzir o número de doentes é fundamental para evitar a circulação do protozoário responsável pela doença, é necessário a criação de ferramentas capazes de avaliar não só a adesão ao tratamento como também a sua eficácia. Com base nos atuais esquemas de tratamento orientados pelo Programa Nacional de Controle da Malária e medicamentos disponibilizados pelo SUS foi desenvolvido e validado, a partir de parâmetros determinados pela RDC N° 27 da ANVISA, um método bioanalítico para a extração e quantificação simultânea de cloroquina, mefloquina, primaquina e lumefantrina, em matriz biológica sangue total, adsorvida em papel filtro. A detecção foi efetuada por CLAE-EM/EM, de maneira remota, em amostras biológicas de pacientes residentes em áreas endêmicas da malária na região Norte do Brasil, em tratamento contra a doença seguindo o protocolo estabelecido pelo Governo Federal. Uma vez o método totalmente desenvolvido e validado, as amostras biológicas originadas dos pacientes foram analisadas por um método de extração simples, rápido, sensível e seletivo, através da técnica de extração líquido-líquido. Todos os parâmetros presentes na legislação em vigor foram alcançados com êxito para os quatro fármacos analitos nos ensaios de validação e nas amostras originadas de pacientes do estudo. Foi possível avaliar parâmetros como absorção e eliminação dos fármacos, o que permitiu verificar a adesão ao tratamento oferecido pelo Governo Federal, através do Sistema Único de Saúde.

Malaria is a neglected disease that has a major impact on public health, specially affecting low income regions. Following the current Brazilian Government's strategy to combat this disease, where reducing the number of patients is essential to avoid the circulation of the malarian parasite, it is necessary to create capable tools of evaluating not only the adherence to treatment but also its effectiveness. Based on the current treatment plans guided by the Programa Nacional de Controle da Malária and their medicines available by SUS, a dried blood spot filter paper bioanalytical methodology using HPLC-MS/MS for the simultaneous extraction and quantification of chloroquine, mefloquine, primaquine and lumefantrine was developed and validated, according to RDC N°. 27 of ANVISA in biological samples of patients residing in malaria endemic areas in the northern region of Brazil, in treatment against the disease following the established protocol by the Federal Government. Once the method was fully developed and validated, the biological samples originated from the patients were analyzed by a simple, fast, sensitive and selective extraction method, using a liquid-liquid extraction technique. All the parameters present in the current legislation were successfully achieved for all the four analyte drugs of the study in the validation tests and in the samples originated from patients of the study, it was possible to evaluate parameters such as absorption and elimination of the drugs, which allowed to verify the adhesion of the treatment offered by the Federal Government, through the Sistema Único de Saúde.
