A leucemia mieloide crônica (LMC) é uma doença de célula-tronco hematopoiética,
caracterizada por anormalidade cromossômica, que resulta em proliferação de células
progenitoras mieloides malignas. O tratamento da LMC foi revolucionado pelo uso de fármacos
inibidores da enzima tirosina- quinase (ITQ), dentre eles o imatinibe (IMA), capaz de
interromper o fenótipo maligno da doença, com melhora significativa da sobrevida livre de
progressão e sobrevida global. O monitoramento da resposta terapêutica norteia a conduta
médica e determina a manutenção ou substituição do ITQ, ou transplante alogênico de células
tronco-hematopoiéticas. Este estudo observacional retrospectivo caracterizou o tratamento de
323 pacientes com diagnóstico de LMC, idade ≥ 18 anos, em tratamento com imatinibe no
período de 2000 a 2018 no Instituto Nacional de Câncer. Observou-se predomínio do sexo
masculino e do diagnóstico na fase crônica da LMC, com mediana de idade de 49,5 (IC95%:
47,6-51,1) e proporção decrescente entre os níveis de escolaridade básica, intermediária e
superior. A adesão ao tratamento medida pelo índice de posse, ou Medication Possession Ratio
(MPR) > 95% no primeiro ano de tratamento com IMA foi de 73,8%. Ao longo de todo o
tratamento, 70,3% dos pacientes foram tratados apenas com IMA em primeira linha de
tratamento e 29,7% em segunda linha com dasatinibe ou nilotinibe. Quanto a resposta
terapêutica, 78,4% tiveram resposta ótima aos 3 meses e 45,7% aos 12 meses. Dentre os
pacientes que alcançaram resposta ótima aos 3 meses, 62,9 % mantiveram resposta ótima aos
12 meses. Dentre os que não alcançaram resposta ótima aos 3 meses e foram avaliados aos 12
meses, 93,2% seguiram sem alcance. A análise multivariada por regressão logística demonstrou
que MPR > 95% e maior escolaridade foram preditores da resposta aos 3 e aos 12 meses nos
pacientes diagnosticados na fase crônica da LMC. Com relação aos eventos de sobrevida, a
Sobrevida Livre de Eventos (SLE) foi de 48,0% (IC95% 42,1% - 53,9%) enquanto a Sobrevida
Global (SG) foi de 76,0% (IC95%: 70,1% - 81,9%) para os pacientes em fase crônica aos 10
anos. A ausência de resposta ótima aos 12 meses e a falha de adesão (MPR≤ 95%) foram fatores
prognósticos para pior SLE, enquanto idade > 60 anos e a ausência de resposta ótima aos 12
meses foram os únicos fatores prognósticos para pior SG. Os resultados deste estudo indicam a
necessidade de acompanhamento multidisciplinar dos pacientes no primeiro ano de tratamento
para promoção da adesão, fator prognóstico no alcance da resposta ótima aos doze meses e
consequentemente melhor SLE e SG.
Abstract
Chronic myeloid leukemia (CML) is a hematopoietic stem cell disease characterized by
a chromosomal abnormality that results in proliferation of malignant myeloid progenitor cells.
The treatment of CML was revolutionized by the use of drugs that inhibit the enzyme tyrosine
kinase (TKI), including imatinib (IMA), capable of interrupting the malignant phenotype of the
disease, with significant improvement in progression-free survival and overall survival. The
monitoring of the therapeutic response guides the medical conduct and determines the
maintenance or replacement of the TKI, or allogeneic hematopoietic stem cell transplantation.
This retrospective observational study was based on a cohort of 323 patients diagnosed with
CML, aged ≥ 18 years, under treatment with imatinib from 2000 to 2018 at the National Cancer
Institute. The median age in the cohort was 49.5 (95%CI: 47.6 - 51.1), and most patients were
in the chronic phase of CML. There was a slight predominance of males, and a decreasing
proportion of basic, intermediate and higher levels of education. Most patients (70.3%) were
treated with only IMA throughout their follow-up, whereas 29.7% received either in dasatinib
or nilotinib as second-line treatment. Treatment adherence measured by Medication Possession
Ratio (MPR) > 95% in the first year of treatment with IMA was 73.8%. As for the therapeutic
response 78.4% had an optimal response at 3 months and 45.7% at 12 months. Among patients
who achieved an optimal response at 3 months, 62.9% maintained an optimal response at 12
months. Among those who failed to reach an optimal response at 3 months, 93.2% continued
failing at 12 months. Multivariate logistic regression analysis showed that MPR > 95% and
higher education were predictors of optimal response at 3 or 12 months in patients diagnosed
in the chronic phase of CML. Regarding survival outcomes at 10 years of follow-up, the EventFree Survival (EFS) was 48.0% (95%CI 42.1% - 53.9%) and the Overall Survival (OS) was
76.0% (95%CI: 70.1% - 81.9%) for patients in the chronic phase, EFS was equal and OS was.
Lack of optimal response at 12 months and poor adherence (MPR ≤ 95%) were the prognostic
factors for worse SLE, whereas age > 60 years and lack of optimal response at 12 months were
the only prognostic factors for worse OS. The results of this study indicate the need for
multidisciplinary follow-up of patients in the first year of treatment to promote adherence, a
prognostic factor in achieving an optimal response at twelve months and, consequently, better
SLE and OS.
