INTRODUÇÃO: A linfoproliferação espontânea, elevação de carga proviral e produção de citocinas proinflamatórias são marcadores imunológicos associados à ativação celular em indivíduos infectados pelo HTLV-1. Tais alterações estão associados a disfunção do sistema imune e ao desenvolvimento de diversas doenças inflamatórias e infecciosas. O vírus Zika (ZIKV) foi identificado no Brasil em 2015 e se tornou um problema de saúde pública devido a sua associação com complicações neurológicas. Pouco ainda é conhecido sobre a resposta imune ao ZIKV, especialmente em indivíduos com co-infecções virais crônicas. É possível que a infecção aguda pelo ZIKV aumente a ativação celular em indivíduos infectados pelo HTLV-1, podendo influenciar no desfecho clínico e progressão para doenças associadas a ambos os vírus. Objetivo: O presente estudo avaliou os efeitos da infecção in vitro pelo ZIKV na ativação celular em pacientes infectados pelo HTLV-1. MÉTODOS: Células mononucleares do sangue periférico (PBMC) de pacientes infectados por HTLV-1 e controles sadios foram infectadas por ZIKV. A confirmação da infecção foi realizada por RT-PCR e Triplex Real Time RT-PCR. A linfoproliferação e o fenótipo celular foram avaliados por citometria de fluxo, a produção de citocinas foi determinada por ELISA e RT-PCR e a carga proviral do HTLV-1 foi quantificada por RT-PCR. RESULTADOS: Foram incluídos vinte indivíduos infectados pelo HTLV-1, 10 assintomáticos e 10 com HAM/TSP. Seis indivíduos não infectados foram incluídos como controles. A presença de ZIKV no sobrenadante da cultura foi detectada em 8 dos 12 indivíduos com HTLV-1 e em todos os indivíduos controles. Não houve aumento da produção de citocinas inflamatórias ou de IL-10 nos sobrenadantes das culturas na presença de ZIKV. A expressão gênica de IL-1 beta e TNF-\03B1 foi estatisticamente maior após 18h de cultura na presença de ZIKV no grupo de indivíduos infectados pelo HTLV-1 com diagnostico de HAM/TSP. Em relação a carga proviral do HTLV-1, houve um aumento significativo nos níveis na presença do ZIKV. CONCLUSÃO: A infecção in vitro pelo ZIKV induziu a aumento da ativação celular com maior produção de citocinas inflamatórias e aumento da carga proviral.
Abstract
INTRODUCTION: Spontaneous lymphoproliferation, increased proviral load and production of proinflammatory cytokines are immunological markers associated with cell activation in individuals infected with HTLV-1. Such changes are associated with dysfunction of the immune system and associated with the development of various inflammatory and infectious diseases. Zika virus (ZIKV) was identified in Brazil in 2015 and has become a public health problem due to its association with neurological complications. Little is known about the immune response to ZIKV, especially in individuals with chronic viral co-infections. It is possible that acute ZIKV infection increases cell activation in individuals infected with HTLV-1, which may influence the clinical outcome and progression to diseases associated with both viruses. OBJECTIVE: The present study evaluated the effects of in vitro ZIKV infection on cell activation in patients infected with HTLV-1. METHODS: Peripheral blood mononuclear cells (PBMC) from patients infected with HTLV-1 and healthy controls were infected with ZIKV. The infection was confirmed by RT-PCR and Triplex Real Time RT-PCR. Lymphoproliferation and cell phenotype were evaluated by flow cytometry, cytokine production was determined by ELISA and RT-PCR and HTLV-1 proviral load was quantified by RT-PCR. RESULTS: Twenty individuals infected with HTLV-1 were included, 10 asymptomatic and 10 with HAM / TSP. Six uninfected individuals were included as controls. The presence of ZIKV in the culture supernatant was detected in 8 of the 12 individuals with HTLV-1 and in all control subjects. There was no increase in the production of inflammatory cytokines or IL-10 in culture supernatants in the presence of ZIKV. Gene expression of IL-1 beta and TNF-\03B1 was statistically higher after 18h of culture in the presence of ZIKV in the group of individuals infected with HTLV-1 diagnosed with HAM / TSP. Regarding the HTLV-1 proviral load, there was a a significant increase in levels in the presence of ZIKV. CONCLUSION: In vitro ZIKV infection induced an increase in cell activation with greater production of inflammatory cytokines and increased proviral load.
