A INFLUÊNCIA DO GÊNERO E DA ATIVIDADE FÍSICA NA ASSOCIAÇÃO ENTRE O DESEMPENHO COGNITIVO E A MORTALIDADE DOS PARTICIPANTES DA COORTE DE IDOSOS DE BAMBUÍ (1997-2011)

O desempenho cognitivo se refere às múltiplas habilidades e aptidões mentais individuais. Com o envelhecimento, observa-se um declínio no desempenho cognitivo, que varia desde alterações normais até aquelas graves que interferem nas atividades da vida diária e na independência dos idosos. A definição do comprometimento cognitivo é complexa e heterogênea, demonstrando o declínio transitório ou permanente na memória, pensamento, concentração ou em outras funções cerebrais que não são explicadas pelo envelhecimento. O reconhecimento do comprometimento cognitivo é de grande importância, porque ele apresenta diferentes causas que incluem eventos adversos de medicamentos, alterações endócrino-metabólicas, deficiências de vitaminas e outros nutrientes, presença de depressão e demências e demais condições. Embora nos últimos anos tenha se observado um aumento do número de estudos que investigam a associação entre o comprometimento cognitivo e a mortalidade, poucos foram aqueles realizados em países de baixa ou média renda. O presente estudo tem como objetivo avaliar a associação entre o comprometimento cognitivo e a mortalidade por todas as causas, bem como o efeito do gênero e da prática de atividade física nessa relação. Assim foram incluídos 1.276 (79,4%) idosos participantes do Projeto Bambuí, que responderam a todas as variáveis envolvidas na presente análise. Os dados de mortalidade foram obtidos através dos relatos de familiares e posteriormente confirmados pelo Sistema de Informações da Mortalidade (SIM) do Ministério de Saúde. Já o comprometimento cognitivo foi estimado através do emprego do Mini Exame do Estado Mental (MEEM), que foi classificado como uma variável categórica ou dicotômica, dependendo da análise empregada. Por outro lado, a prática de atividade física foi definida pela realização de qualquer exercício por um período de 20-30 minutos e com uma frequência de 3 ou mais vezes em uma semana durante os últimos noventa dias. O modelo de riscos proporcionais de Cox foi utilizado para estimar a associação do comprometimento cognitivo e a mortalidade por todas as causas. Foram realizadas análises não-ajustadas e ajustadas por fatores de confusão em toda amostra e nas amostras estratificadas por gênero e pela prática da atividade física. Os principais resultados encontrados foram a influência do gênero e da atividade física na associação entre o comprometimento cognitivo e a mortalidade por todas as causas. Assim observou-se que idosos com comprometimento cognitivo do gênero masculino (HR: 2,30; IC 95%: 1,12-4,74) e que não fazem atividade física (1,38; IC 95%: 1,09-1,75) apresentam um risco maior para mortalidade para todas as causas. Entretanto, pesquisas futuras são necessárias, com o objetivo de replicar os achados desse estudo, além de determinar os mecanismos que estão por trás da influência do gênero e da prática de atividade física na associação entre o comprometimento cognitivo e a mortalidade por todas as causas.
Abstract
Cognitive performance refers to multiple individual mental skills and abilities. With aging, there is a decline in cognitive performance ranging from normal to severe changes that interfere with activities of daily living and independence of the elderly. The definition of cognitive impairment is complex and heterogeneous, demonstrating transient or permanent decline in memory, thinking, concentration or other brain functions that are not explained by aging. The recognition of cognitive impairment is of great importance because it has different causes that include adverse drug events, endocrine-metabolic changes, deficiencies in vitamins and other nutrients, the presence of depression and dementia and other conditions. Although in recent years there has been an increase in the number of studies investigating the association between cognitive impairment and mortality, few have been carried out in low- or middle-income countries. The present study aims to assess the association between cognitive impairment and mortality from all causes and the effect of gender and physical activity in this relationship. Thus, 1,276 (79.4%) elderly participants of the Bambuí Project were included, who answered all the variables involved in the present analysis. Mortality data were obtained through reports from family members and later confirmed by the Ministry of Health's Mortality Information System (SIM). Cognitive impairment was estimated through the use of the Mini Mental State Examination (MMSE), which was classified as a categorical or dichotomous variable depending on the analysis employed. On the other hand, the practice of physical activity was defined as the performance of any exercise for a period of 20-30 minutes and with a frequency of 3 or more times in a week during the past ninety days. Cox's proportional hazards model was used to estimate the association of cognitive impairment and all-cause mortality. Unadjusted analyzes were performed and adjusted for confounding factors across the sample and samples stratified by gender and physical activity. The main results found were the influence of gender and physical activity on the association between cognitive impairment and mortality from all causes. Thus, it was observed that elderly people with cognitive impairment of the male gender (HR: 2.30; 95% CI: 1.12-4.74) and who do not do physical activity (1.38; 95% CI: 1.09- 1.75) have a higher risk of mortality from all causes. However, future research is necessary in order to replicate the findings of this study, in addition to determining the mechanisms behind the influence of gender and the practice of physical activity in the association between cognitive impairment and mortality from all causes.
