INTRODUÇÃO: O câncer colorretal é a segunda causa de morte por câncer no Brasil, o diagnóstico é tardio, com poucas chances de cura. A morbimortalidade pode ser reduzida com programas de rastreamento. OBJETIVO: Analisar estratégias para rastreamento do câncer colorretal, visando elaborar recomendação para o Sistema Único de Saúde. MÉTODOS: Overview de revisões sistemáticas com busca em sete bases de dados da literatura científica entre 09 a 11/2016. RESULTADOS: De 760 revisões, 28 atenderam aos critérios de inclusão e dez foram eleitas por apresentar média e alta qualidade. Não foram encontradas revisões ou estudos primários em países de média renda. Os achados mostraram que o rastreamento da população acima de 45 anos, com teste de sangue oculto nas fezes baseado em guaiaco reduz a mortalidade em 16%; eventos adversos relacionaram-se à investigação complementar. A principal barreira de implementação reside na adesão ao teste de rastreio, influenciada por necessidade de dieta, manuseio de fezes, forma de distribuição do teste e intervenções entre a equipe de saúde e população. Barreiras estruturais também foram relatadas. A participação do indivíduo na realização do teste imunoquímico de sangue oculto nas fezes foi maior quando comparado ao teste baseado em guaiaco (RR 1,21; IC 95% 1,09-1,33). CONCLUSÃO: O rastreamento com testes não invasivos reduz a mortalidade por câncer colorretal. Questões de implementação são decisivas para o impacto do rastreamento nos indicadores de saúde. Programas organizados de base populacional produzem maior equidade de acesso.

BACKGROUND: Colorectal cancer is the second leading cause of death by cancer in Brazil; diagnosis is often late, with lower chances of cure. Morbidity and mortality can be reduced by screening programmes. OBJECTIVE: Evaluate strategies for colorectal cancer screening, aiming at purposing a policy for the Unified Health Care System. METHODS: Overview of systematic reviews; seven literature databases were searched, from 09/2016 and 11/2016. RESULTS: From 760 reviews, 28 met the inclusion criteria and 10 were included by achieving moderate and high methodological quality. The search did not retrieve any studies performed in middle income countries. The findings supported that screening adults over 45 years old with the guaiac fecal occult blood test reduces mortality by 16%; adverse events were related to further investigation. The main barrier to implement screening is related to population participation rate, affected by dietary restrictions, feces manipulation, ways of delivering the test and interventions between the health care provider and the target population. Structural barriers were also reported. Participation rates were higher for the immunochemical test compared to guaiac (RR 1,21; IC 95% 1,09-1,33). CONCLUSION: Screening for colorectal cancer with non-invasive tests reduces specific mortality. Implementation issues are key to achieve health indicators. Population based screening programmes enhance equity of access.
