A assistência farmacêutica é considerada um elemento estruturante da rede de atenção oncológica. A compreensão sobre sua organização é essencial, pois, favorece a identificação de oportunidades para melhoria das ações implementadas em sistemas integrados de atenção à saúde. Este estudo objetiva analisar a organização da assistência farmacêutica na rede de atenção oncológica no âmbito do Sistema Único de Saúde, visando identificar seus limites e possibilidades. Para tanto, foram combinados métodos com o intuito de fornecer uma visão ampliada sobre o objeto investigado, tendo como condição marcadora o câncer de mama. O estudo foi realizado em cinco etapas: 1) análise documental da política de atenção oncológica; 2) análise da estrutura assistencial e do perfil de atendimento em oncologia; 3) análise do fluxo de atendimento ambulatorial de quimioterapia; 4) caracterização dos municípios que dispõem de serviços de quimioterapia; 5) estudo de casos múltiplos em municípios selecionados, envolvendo entrevista com gestores da rede de atenção oncológica e da assistência farmacêutica municipal, e com profissionais das equipes multiprofissionais de terapia antineoplásica de serviços de quimioterapia. A pesquisa se utilizou da teoria da estruturação de Giddens, devido ao seu potencial explicativo e analítico, e do delineamento ecológico, para gerar hipóteses e favorecer a seleção dos municípios investigados, ampliando a capacidade de interpretação dos resultados. Os principais limites verificados foram: insuficiência da rede assistencial e ausência de indução financeira para sua expansão; dificuldades de acessibilidade geográfica para realização de quimioterapia; disparidade na oferta de serviços em oncologia; baixa integração dos diversos níveis de atenção, em prol da prevenção e controle do câncer; subfinanciamento da atenção oncológica; dificuldades em garantir acesso e a continuidade do tratamento; e ausência de mensuração dos desfechos clínicos. As possibilidades identificadas envolvem: a sistematização de uma política de assistência farmacêutica em oncologia no âmbito do Sistema Único de Saúde; o aperfeiçoamento do modelo de gestão e organização da rede de atenção oncológica, e da própria assistência farmacêutica; e o subsídio a ações para melhorar o desempenho da assistência farmacêutica e a qualidade do cuidado em oncologia. A análise permitiu traçar um panorama sistêmico da organização da assistência farmacêutica em oncologia no Sistema Único de Saúde.
Abstract
Pharmaceutical services are acknowledged as an essential structural element for the cancer care network and understanding how this network is organized helps to identify opportunities for improvement of health care in comprehensive systems. This study aims to analyze the organization of pharmaceutical services in cancer care network of the Brazilian National Health System, to identify limits and possibilities for action, using breast cancer as a marker condition. Different methods were combined to offer a broad view of the issue. Research was undertaken in five steps: 1) analysis of brazilian oncology care policy documentation; 2) analysis of the structure and profile of oncology care in Brazil; 3) analysis of outpatient chemotherapy origin-destination flows; 4) characterization of municipalities that harbor chemotherapy facilities; 5) multiple case study in selected municipalities, involving in-depth interviews with municipal cancer care network and pharmaceutical services managers, professionals involved in chemotherapy care teams in chemotherapy facilities. The study adopted Giddens' structuration theory because of its analytical and explanatory potential as well as ecological study methods to generate hypotheses and selection of municipalities, and to amplify analysis and interpretation of results. Main limits were insufficiency of care network and lack of financial backing for network expansion, barriers in geographical accessibility for chemotherapy, disparities in offer of chemotherapy services, inadequate integration among different levels of cancer prevention and care, underfinancing of oncology care, hindrances for continuity of care and access to treatment, absence of measurable clinical outcomes. Possibilities for action involve implementation of a pharmaceutical services cancer policy in Brazilian National Health System, refinement of organization and management of cancer care network and of pharmaceutical services, and support for implementation of actions to better pharmaceutical services performance and overall quality of cancer care. The analyses offered a systemic outlook of the organization of pharmaceutical services for cancer in Brazilian National Health System.
