O câncer de próstata acomete principalmente os idosos e geralmente apresenta um curso indolente. A maioria dos indivíduos diagnosticados com doença localizada não morre pela neoplasia, mas em decorrência de causas competitivas. Esta dissertação teve por objetivos: 1) conhecer as causas múltiplas de óbito por câncer de próstata (CP); e 2) determinar a frequência em que o CP é selecionado como causa básica de óbito quando mencionado apenas como causa contribuinte, identificando as causas básicas originalmente declaradas. Para tanto, foi realizado um estudo descritivo utilizando dados do Sistema de Informações de Mortalidade do período de 2006 a 2015 fornecidos pela Secretaria de Estado de Saúde do Rio de Janeiro. Foram analisadas todas as declarações de óbito (DO) que mencionaram o CP em qualquer linha. O programa Tabulador de Causas Múltiplas de Morte (TCM) foi utilizado para produzir tabelas de referência cruzada de causas básicas e associadas segundo capítulos e grupos de causas da CID-10. Nos óbitos em que o CP foi a causa básica, determinou-se a frequência em que foi mencionado nas Partes I ou II da DO, segundo faixa etária e região de residência (capital/interior). Quando esta foi causa contribuinte, apresentou-se a distribuição de frequência das causas básicas declaradas. O CP foi mencionado em 15.595 óbitos, com idade mediana de 77 anos. Foi a causa básica em 86% destes, sendo as causas associadas mais frequentes os sintomas, sinais e achados anormais de exames clínicos e laboratoriais (34,2%), neoplasias secundárias ou localmente mal definidas (28,3%), insuficiência respiratória aguda ou não especificada (22,2%), septicemias (18,3%), insuficiência renal (12,3%) e pneumonias (11,7). Quando o CP foi a causa associada, as principais causas básicas foram as doenças isquêmicas do coração (13,5%), cerebrovasculares (10%) e diabetes mellitus (7,8%). As pneumonias não figuraram entre as causas básicas. Dos 13.390 óbitos por CP, a neoplasia fora declarada como contribuinte em 14,8%. Nesses óbitos, as causas básicas declaradas com maior frequência foram: pneumonias (28,1%), infecção do trato urinário (15,0%), causas mal definidas (13,3%), septicemias (10,3%) e insuficiência renal (6,2%). A análise de todas as causas informadas na DO permitiu identificar causas associadas aos óbitos por CP que podem ser alvo de prevenção e controle. As regras de seleção de causa básica podem contribuir para a atribuição ao CP de óbitos por outras causas, especialmente as pneumonias.
Abstract
Prostate cancer mainly affects the elderly and usually presents an indolent course. Most individuals diagnosed with localized disease do not die from the neoplasm, but from competitive causes. This dissertation aimed to: 1) know the multiple causes of death due to prostate cancer (PC); and 2) determine the frequency at which PC is selected as the underlying cause of death when mentioned only as a contributory cause, identifying the underlying causes originally stated. For that, a descriptive study was carried out using data from the Mortality Information System for the period from 2006 to 2015 provided by the State Department of Health of Rio de Janeiro. We analyzed all death certificates (DC) that mentioned PC in any line. The Multiple Causes of Death (TCM) program was used to produce cross-reference tables of underlying and associated causes according to ICD-10 chapters and groups. For deaths with PC as the underlying cause, we estimated the frequency of mentions in Parts I or II of the DO, according to age group and region of residence (capital/interior). When it was the contributory cause, the distribution of the declared underlying causes was presented. PC was mentioned in 15,595 deaths, with a median age of 77 years. It was the main cause in 86% of these. Symptoms, signs and abnormal findings of clinical and laboratory tests (34.2%), secondary or locally ill-defined neoplasms (28.3%), acute respiratory failure (22.2%), septicemia (18.3%), renal insufficiency (12.3%) and pneumonia (11.7) were the most frequent associated causes. When PC was the associated cause, the main underlying causes were ischemic heart diseases (13.5%), cerebrovascular diseases (10%) and diabetes mellitus (7.8%). Pneumonia was not among the underlying causes. PC was declared as contributory cause in 14.8% of the 13,390 deaths per PC. In these deaths, the most frequently reported underlying causes were pneumonia (28.1%), urinary tract infection (15.0%), ill-defined causes (13.3%), septicemia (10.3%), and renal insufficiency (6.2%). The analysis of all the causes informed in the DC allowed the identification of associated causes in deaths by PC that can be the target of prevention and control. The rules of selection of underlying cause can contribute to the attribution to PC of deaths from other causes, especially pneumonia.
