O estudo teve como objetivo analisar a colaboração interprofissional entre trabalhadores de diferentes serviços no cuidado em saúde mental, considerando a interdependência entre aspectos estruturais, organizacionais e intersubjetivos, a partir da experiência do Núcleo Ampliado de Saúde da Família e Atenção Básica \2013 NASF-AB em um município do estado do Rio de Janeiro. Trata-se de uma pesquisa qualitativa, baseada no paradigma construtivista, que se pautou na realização de um estudo de caso único. As estratégias metodológicas utilizadas foram: observação participante, grupos focais narrativos e entrevistas semiestruturadas. O material produzido ao longo do trabalho de campo foi analisado a partir da perspectiva da hermenêutica filosófica; do referencial teórico de autores do campo da saúde pública/coletiva que discutem a temática da colaboração interprofissional; do referencial teórico sobre o imaginário social/organizacional da psicossociologia francesa e da filosofia de Cornelius Castoriadis; e a partir da contribuição da psicodinâmica do trabalho a respeito dos limites e possibilidades para a cooperação. Os resultados apontam para uma indissociabilidade dos elementos estruturais, organizacionais e intersubjetivos na colaboração interprofissional. Destaca-se um histórico de matriciamento em saúde mental e iniciativas voltadas para a desmedicalização do cuidado dos usuários que mobilizava os profissionais no sentido de uma prática colaborativa voltada para a promoção da saúde mental. Por outro lado, foi identificada, também, um desafio quanto ao compartilhamento da responsabilização entre as equipes NASF-AB/Estratégia de Saúde da Família - ESF na coordenação do cuidado, em especial no caso de usuários com problemas mais complexos. A partir do referencial do imaginário social, foi possível identificar um imaginário motor presente no relato das equipes a respeito do histórico no trabalho de apoio matricial e de sua expertise na realização de Práticas Integrativas e Complementares-PICs, bem como um imaginário regido pelo signo da impotência de ação compartilhado entre as equipes NASF-AB/ESF no que diz respeito ao acompanhamento longitudinal dos usuários. Destacam-se, também, que os tensionamentos na relação entre as equipes estiveram, predominantemente, relacionados à organização dos processos de trabalho. Algumas regras construídas no contexto das unidades com o intuito de induzir a cooperação, tinham um efeito paradoxal de gerar uma participação desimplicada por uma parcela dos trabalhadores. Por fim, mostra-se importante refletir sobre as relações de confiança no trabalho como elemento fundamental para a colaboração, relações estas que podem ser influenciadas pela configuração de um ambiente organizacional, em que vigore a negociação entre os trabalhadores e entre estes e os gestores, capaz de aglutinar interesses e lidar com os inevitáveis conflitos.
Abstract
The study aimed to analyze the interprofessional collaboration between workers from different services in mental health care, taking into consideration the interdependence between structural, organizational and intersubjective aspects, based on the experience of the Expanded Center of Family Health and Primary Care (NASF-AB) in a municipality in the state of Rio de Janeiro. It is a qualitative research, based on the constructivist paradigm, which was based on the realization of a single case study. The methodological strategies used were: participant observation, narrative focus groups and semi-structured interviews. The material produced during the fieldwork was analyzed from the perspective of philosophical hermeneutics; the theoretical framework of authors in the field of public health who discuss the theme of interprofessional collaboration; the theoretical framework on the social / organizational imagery of French psychosociology and philosophy by Cornelius Castoriadis; and from the contribution of psychodynamics of work regarding the limits and possibilities for cooperation. The results point to an inseparability of the structural, organizational, and intersubjective elements in interprofessional collaboration. We highlight the background in mental health support and initiatives aimed at the de-medicalization of user care that mobilized professionals towards a collaborative practice aimed at mental health promotion. On the other hand, a challenge was also identified regarding the sharing of responsibility between the NASF-AB / Primary Care Teams (ESF) in the coordination of care, especially in complex mental health problems. From the referential of the social imaginary, this provided to identify an imaginary driving in the teams' report regarding the history of matrix support work and their expertise in carrying out complementary and integrative health practices, as well as an imaginary governed by the sign the powerlessness of action shared between the NASF-AB / ESF teams regarding longitudinal follow-up of patients. It is also noteworthy that the tension in the relationship between the teams was predominantly related to the organization of work processes. Some rules built in the context of the primary health care units with the aim of inducing cooperation, had a paradoxical effect of generating unimplicated participation by some of the workers. Finally, it is important to reflect on the relationships of trust at work as a fundamental element for collaboration, relationships that can be influenced by the configuration of an organizational environment, in which the negotiation between workers and between them and managers prevails, able to bring together interests and deal with the inevitable conflicts.
