As iniquidades socioeconômicas têm forte impacto nos desfechos observados na saúde pública, aumentando níveis de morbidade e mortalidade. Maiores níveis de equidade são, portanto, indispensáveis à boa saúde da população. Contudo, é desafiador definir a equidade e mais ainda encontrar meios de operacionalizá-la. Para chegar à definição, este trabalho revisa as teorias de Justiça de John Rawls, autor consagrado no tema; de Amartya Sen, contemporâneo e crítico de Rawls; e da Bioética Latino-Americana, que propõe contrapontos ao pensamento liberal estadunidense dos autores anteriores. A partir dessa revisão, analisam-se as formas de mensurar a equidade, operacionalizando-a, por meio de estudos do campo da saúde pública. Constata-se o caráter ético da equidade, o qual traz desafios tanto à definição nas políticas públicas, resultando em polissemia, quanto à aplicação prática do conceito. Observam-se os efeitos dessas dificuldades nas políticas nacionais de saúde, que estabelecem diferentes descrições e poucos indicadores, entendendo a equidade como o comparativo entre as situações de saúde das \201Cminorias\201D e das \201Cmaiorias\201D. O Programa Bolsa-Família mostrou-se a principal política brasileira de promoção da equidade socioeconômica em tempos recentes, buscando eliminar a pobreza intergeracional - com relativo grau de sucesso, porém com necessidades de aprimoramento para cumprir com o objetivo. A revisão da literatura internacional das estratégias de redução da pobreza, utilizando o método das matrizes, identifica e compara diferentes categorias de intervenções (programas de transferência de renda; microcrédito; serviços de saúde; preservação ambiental; emprego), destacando seus pontos fracos e fortes. Constata-se que o tipo de estratégia é menos relevante para o sucesso da intervenção que a forma como é aplicada, a qual requer embasamento no paradigma da complexidade, participação comunitária e ênfase na promoção do capital humano e social. Conclui-se que o desenvolvimento ético é indispensável para efetivar a equidade na sociedade, condição sem a qual as medidas de redução da pobreza se convertem em ações paliativas para suprimir as iniquidades, até que se desenvolva senso mais elevado de justiça.
Abstract
Socioeconomic inequities have a strong impact on the outcomes observed in public health, increasing levels of morbidity and mortality. Greater levels of equity are therefore indispensable to the good health of the population. However, it is challenging to define equity and even more to find ways to operationalize it. To reach the definition, this work reviews the theories of Justice of John Rawls, author dedicated to the subject; by Amartya Sen, contemporary and critic of Rawls; and of Latin American Bioethics, which proposes counterpoints to the American liberal thinking of the previous authors. Based on this review, we analyze the ways of measuring equity, making it operational, through studies in the field of public health. The ethical character of equity is presented, which challenges both the definition in public policies, resulting in polysemy, and the practical application of the concept. We highlight the effects of these difficulties on national health policies, which establish different descriptions and few indicators, conceptualizing equity as the comparative between health situations of "minorities" and "majorities". The Bolsa Família Program was the main Brazilian policy to promote socioeconomic equity in recent times, seeking to eliminate intergenerational poverty \2013 with a relative degree of success, but requiring improvements to reach the proposed goal. The review of the international literature on poverty reduction strategies, using the matrix method, identifies and compares different categories of interventions (income transfer programs, microcredit, health services, environmental preservation, employment), highlighting their strengths and weaknesses. It is noted that the type of strategy is less relevant to the success of the intervention than the way it is applied, which requires a basis in the paradigm of complexity, community participation and emphasis on the promotion of human and social capital. It is concluded that ethical development is indispensable for effecting equity in society, a condition without which poverty reduction measures become palliative actions to suppress inequities until a higher sense of justice arises.
