São inúmeros os benefícios do aleitamento materno; suas consequências, de curto, médio e longo prazos. O aleitamento materno afeta diretamente os padrões de saúde e de mortalidade das populações: quanto mais tardiamente o aleitamento materno é iniciado, maiores as chances de óbito neonatal causado por infecções, sendo ainda maiores os riscos para recém-nascidos que recebam outro alimento além do leite materno. Considerando a importância do aleitamento materno, na presente tese são apresentados três estudos analíticos, que tiveram por base os dados da pesquisa "Nascer no Brasil", representativa nacionalmente, na qual foram entrevistadas 23.894 mulheres, nas cinco regiões do país, entre fevereiro de 2011 e outubro de 2012. Nos três estudos, foram utilizados modelos de regressão logística múltipla. O primeiro artigo evidencia que bebês nascidos em hospitais da Iniciativa Hospital Amigo da Criança têm maiores chances de ser amamentados exclusivamente no hospital, seja ele público, conveniado ao SUS ou privado. O segundo artigo evidencia que mães com sobrepeso ou obesas têm menores chances de amamentar na primeira hora de vida quando não há apoio social. Por fim, no terceiro artigo, observou-se menores chances de aleitamento materno para bebês que nascem entre a 37a e a 38a semanas gestacionais, classificados como a termo precoce, sendo os resultados ainda piores para bebês nascidos por cesariana. Conclui-se que os resultados da tese reforçam evidências prévias de que, em ambiente hospitalar, determinantes contextuais se associam mais fortemente às práticas de aleitamento materno, ou seja, as mães têm pouca ou nenhuma autonomia sobre as decisões acerca de sua saúde e de seus bebês. Logo, recomenda-se o fortalecimento e a expansão das políticas de amamentação implementadas até então e a elaboração de novas estratégias que incluam os grupos mais vulneráveis, incluindo aqueles apontados pelos estudos da presente tese.
Abstract
The benefits of breastfeeding are numerous and it has short, medium and long term consequences. Breastfeeding directly affects health and mortality patterns. The later breastfeeding is initiated, the greater the chances of neonatal death caused by infections, with even greater risks for newborns receiving food other than breast milk. Considering the importance of breastfeeding, in this thesis three analytical studies are presented, which were based on data from the survey "Birth in Brazil", nationally representative, in which 23,894 women were interviewed, in the five regions of the country, between February 2011 and October 2012. In the three studies, multiple logistic regression models were used. The first article shows that babies born in hospitals of the Baby-Friendly Hospital Initiative are more likely to be breastfed exclusively during the hospital stay, whether these hospitals are in public, mixed or private, which indicates the need to strengthen and expand the initiative in the country. The second article shows that overweight or obese mothers are less likely to breastfeed in the first hour after birth when there is no social support. Finally, in the third article, babies born between the 37th and 38th gestational weeks, classified as early term, had lower chances of being breastfed within the first hour after birth, and the odds were even lower for babies born by cesarean section. The results of the thesis reinforce previous evidence that, in a hospital environment, contextual determinants are more strongly associated with breastfeeding practices, that is, mothers have little or no autonomy over decisions about their health and their babies. Therefore, it is recommended to strengthen and expand the existing breastfeeding policies to develop new strategies that include the most vulnerable groups, including those indicated by the studies of this thesis.
