DIAGNÓSTICO E CARACTERIZAÇÃO DE LEISHMANIA SPP. EM AMOSTRAS CLÍNICAS DE PACIENTES COM LEISHMANIOSE CUTÂNEA NO ESTADO DE RORAIMA

A leishmaniose tegumentar (LT) é uma doença endêmica no Brasil, sendo a região Norte responsável pelo maior número de casos no país. Nesse contexto, destaca-se o contínuo e crescente números de casos registrados no SINAN no estado de Roraima em humanos, sem registro efetivamente de estudos que identificasse as espécies de Leishmania que acometem a população. O presente estudo se propôs a caracterizar as espécies de Leishmania circulantes entre humanos em amostras isoladas de pacientes com leishmaniose cutânea em áreas endêmicas do Estado de Roraima no período de 2016 a 2018. Foram coletadas 262 amostras de pacientes com suspeita clínica de LT e submetidas ao exame parasitológico direto (PDL) e caracterização molecular por PCR, utilizando iniciadores para os alvos kDNA, ITS-1. 81,6% pertenciam a pacientes do sexo masculino. A apresentação clínica cutânea predominante foi a presença de uma única lesão (80,3%). Das 262 amostras, foram selecionadas 76 amostras positivas nas PCR-ITS-1 e PCR-kDNA, cujo sequenciamento gênico possibilitou a caracterização de oito espécies diferentes: L. (Viannia) braziliensis (30,3%), L. (L.) amazonensis (18,4%), L.(V) guyanensis (5,3%), L. (V) naiffi (1,3%), L.(V) shawi (1,3%), L.(V) panamensis (13,2%), L. (V) utingensis (1,3%), L.(V) lindenbergi (1,3%), entretanto,25% das amostras apresentaram perfis de Leishmania sp, não sendo possível a identificação das espécie. Epidemiologicamente, este estudo é pioneiro em revelar as espécies de Leishmania circulantes em Roraima em humanos. Além das Leishmania já identificadas no Brasil, nós caracterizamos duas espécies novas, L. panamensis e a L. (V) utingensis, o que ressalta a importância da investigação sistemática dos isolados para que possamos conhecer a real etiologia dos casos de LT no país.

Cutaneous leishmaniasis (CL) is an endemic disease in Brazil, being the northern region responsible for the largest number of cases in the country. In this context, we highlight the continuous and growing number of cases registered in SINAN in the state of Roraima in humans, with no effective record of studies that identified the species of Leishmanias that affect the population. The present study aimed to characterize the species of Leishmania spp. circulating among humans in isolated samples from patients with cutaneous leishmaniasis in endemic areas of the State of Roraima in the period from 2016 to 2018. 262 samples were collected from patients with clinical suspicion of CL and subjected to direct parasitological examination (PDL) and molecular characterization by PCR using primers for the kDNA, ITS-1 targets. 81.58% belonged to male patients. The cutaneous clinical form was predominant in a single lesion (80.26%). Of the 262 samples, 76 positive samples were selected in PCR-ITS-1 and PCR-kDNA, which through sequencing, were able to characterize eight different species: L. (V) braziliensis (30.27%), L. ( L) amazonensis (18.42%), L. (V) guyanensis (5.25%), L. (V) naiffi (1.31%), L. (V)shawi (1.31%), L.(V)panamensis ( 13.17%), L.(V) utingensis (1.31%), L. (V)lindenbergi (1.31%) and 25% of the samples presented profiles of Leishmania sp. Epidemiologically, the study is a pioneer in revealing the species of leishmanias circulating in Roraima by molecular biology in humans, and thus, further investigations on vectors, hosts, the incidence of the disease, the etiological agents, as well as intensification of surveillance and control of CL in the state.
