O principal agente de meningite fúngica no mundo é uma levedura capsulada que
apresenta duas espécies distintas: Cryptococcus neoformans e Cryptococcus gattii, que
fazem parte do complexo C. neoformans. Com o aparecimento da AIDS, houve expressivo
aumento na incidência da criptococose. Posteriormente, o uso de antifúngicos profiláticos e
a introdução da HAART levaram à redução desta incidência. Objetivamos determinar o
perfil epidemiológico da meningite criptocócica no Estado do Rio de Janeiro no período de
1994 a 2004, com base nos dados da Assessoria de Meningite, setor do Centro de
Vigilância Epidemiológica da SSE-RJ e, também, avaliar em que medida o perfil
epidemiológico disponível no sistema nacional (SINAN) refletiria as ocorrências da
meningite criptocócica no ERJ, no período de 2000 a 2004. Um total de 696 casos novos de
meningite criptocócica foram registrados no período de 1994 a 2004, representando uma
taxa de incidência média anual de 0,45 casos por 100.000 habitantes. Houve predominância
do sexo masculino no período, a média de idade foi de 35,9 anos, a AIDS foi a única
enfermidade preexistente registrada, estando presente em 61,2% do total de pacientes, e a
letalidade foi de 51,8%. No período, não se observou diminuição na incidência após a
introdução da HAART. Entretanto, considerando que a incidência de AIDS registrou
aumento no período no ERJ e que não foi identificado aumento na incidência da meningite
criptocócica, isto sugere que poderia ter havido redução da meningite criptocócica entre os
pacientes infectados pelo HIV. A meningite criptocócica, como as demais meningites, deve
ser notificada através da Ficha de Investigação Individual de Meningites do Sistema de
Informação de Agravos de Notificação. O banco de dados do SINAN foi comparado com
um novo banco, denominado “banco ampliado”, composto pelos casos de meningite
criptocócica do SINAN, da Assessoria de Meningite e dos registros do laboratório do
Instituto Estadual de Infectologia São Sebastião. O SINAN captou 65,7% dos casos
presentes no “banco ampliado”, entre 2000 e 2004. A incidência de meningite criptocócica
está subestimada no SINAN, mas o perfil dos casos notificados neste sistema reflete o
perfil do total de casos, considerando o “banco ampliado” como próximo ao universo dos
pacientes com meningite criptocócica no Estado. Não foram observadas diferenças na faixa
etária, na proporção homens: mulheres, na letalidade ou no percentual de recidivas.

The most common agent of fungal meningitis worldwide is a capsulated yeast
classified into two species: Cryptococcus neoformans and Cryptococcus gattii, part of the
C. neoformans species complex. As the AIDS epidemic developed there was a marked
increase of incidence with a subsequent fall as a result of the use of prophylactic antifungal
drugs and the introduction of HAART. We aimed to evaluate the epidemiology of
cryptococcal meningitis in Rio de Janeiro, between 1994 and 2004, based on data from the
Assessoria de Meningite, from the Centro de Vigilância Epidemiológica da SSE-RJ, and
also to assess in what extent the cases of cryptococccal meningitis reported to the SINAN
(National Information System of Notifiable Diseases) represented the occurrence of
cryptococcal meningitis in the State of Rio de Janeiro, from 2000 to 2004. Six hundred and
ninety-six cases of cryptococcal meningitis were reported from 1994 to 2004, with a mean
incidence of 0.45 per 100000 persons. Male sex predominated; mean age was 35.9 years;
AIDS was the only underlying disease, reported in 61.2% of the cases; the mortality was
51.8%. No decline in incidence was observed after the introduction of HAART. However,
as there had been a rising incidence of AIDS during the period and no rise was observed in
the incidence of cryptococcosis, this may suggest a decline amongst patients with HIV
infection. Cryptococcal meningitis must be reported to the National Information System of
Notifiable Diseases (SINAN). The database from SINAN was compared to a new database,
called “expanded database”, formed by cases from three sources: SINAN, Assessoria de
Meningite and the laboratory of the Instituto Estadual de Infectologia São Sebastião. From
2000 to 2004, in the SINAN database there were 65.7% of the cases of the “expanded
database”. Cryptococcal meningitis is underestimated at SINAN database, but the patients’
characteristics are similar to those of the total number of cases. No differences were found
related to age, sex, mortality or relapse rate.
