Introdução: O sobrepeso e a obesidade vêm crescendo em prevalência entre indivíduos idosos, e a relação entre a mudança da composição corporal e síndromes geriátricas específicas recentemente vem atraindo a atenção de pesquisadores, pelo alto impacto econômico da obesidade na população idosa, tanto em recursos humanos quanto materiais (KUO et al, 2006). Objetivo: Analisar parâmetros da composição corporal
determinantes na qualidade de vida de indivíduos idosos. Amostra: Foram avaliados 144 indivíduos idosos (acima de 60 anos), ambos os sexos, participantes do projeto social promovido pela Secretaria Municipal de Esportes e Lazer/Câmara Municipal do Rio de Janeiro, no Centro Esportivo Miécimo da Silva, de 2005 a 2007, classificados como fisicamente ativo pelo questionário internacional de atividade física (IPAQ).
Metodologia: Nesta pesquisa descritiva correlacional foi avaliada a qualidade de vida através do questionário WHOQOL-OLD (WHO, 2004), e os seguintes parâmetros de composição corporal: estatura, peso, medidas circunferenciais, dobras cutâneas, Índice de Massa Corporal (IMC), Relação Cintura-Quadril (RCQ), circunferência de cintura (CC), e nível de atividade física (testes Baecke e IPAQ). O presente estudo observou
dois momentos distintos no tratamento estatístico: o primeiro relativo à descrição dos dados segundo as variáveis experimentais, observando os protocolos de avaliação da normalidade (Komogorov-Smirnov) destas e suas potências de tamanho amostral, além do cálculo dos valores médios e demais parâmetros estatísticos de quantificação das dimensões variacionais; o segundo momento tratou da parte inferencial do estudo
buscando testar o corpo de hipóteses segundo o teste não paramétrico Qui-quadrado de Pearson para verificação de associação (p<0,05). O método de classificação de Cluster também foi utilizado para variáveis que observam distribuições sob uma curva normal e de conteúdo contínuo paramétrico, resultando em divisão da amostra em três grupos.
Resultados: Dos idosos avaliados, 67% apresentaram obesidade, e observou-se que aproximadamente 50 % do grupo avaliado apresentou índices elevados de RCQ e CC. Este maior acúmulo de gordura na região central do corpo está relacionado à maior incidência de disfunções crônicas degenerativas (CABRERA et al, 2005). O escore total da qualidade vida no WHOQOL-OLD apresentado pelos idosos deste estudo foi de
13,17 (57,28%) de satisfação. Este escore se deve principalmente às respostas das facetas 2, 3, 4 e 6. As mesmo tempo, o escore 1 “funcionamento sensório” e o escore 5 “morte e morrer” tiveram os mais baixos escores. O grupo de Cluster que apresentou uma maior prevalência em relação aos demais (grupo 2) possui um escore intermediário
de atividade física enquanto que o de menor percentual relativo (grupo 3) possui os maiores níveis de atividade física. Ao correlacionar as variáveis, observa-se uma associação significativa (p=0,017) para p<0,05 entre as variáveis sexo e RCQ. A dobra subescapular também apresenta associação significativa (p=0,001) para p < 0,05 com a variável IMC (p=0,006) e CC. Observa-se ainda associação significativa para p<0,05 entre as variáveis RCQ, nível de atividade física (Baecke) e o escore total de qualidade
de vida, o que representa que indivíduos com menores valores de RCQ apresentam simultaneamente melhores níveis de atividade física e melhor qualidade de vida.
Conclusão: A qualidade de vida global e suas diferentes facetas sugerem duas formas diferentes de avaliação da qualidade de vida de idosos: como um todo (domínio global) ou seus aspectos isolados (facetas).
Abstract
Background: The prevalence of overweight and obesity is increasing between elderly although this relation between body composition and specific geriatric syndromes is just recently attracting researchers as impact of obesity is expensive in both natural and material resources (KUO et al, 2006). Purpose: To analyze body composition in elderly in quality of life and health promotion. Sample: 144 elderly, minimum age 60 years
old; both gender; part of the social project promoted by Leisure and Sports Municipal Secretary/Municipal Chamber of Rio de Janeiro, from 2005 to 2007, classified as physically actives by IPAQ. Methods: This research caracterizes as correlational descriptive. Quality of Life was evaluated through WHOQOL-OLD and body composition by stature, weight, circumference measures, coetaneous plait, BMI (Body
Mass Index), BHR (Belt-Hip Relation), WC (waist circumference); and physical activity level were evaluated by Baecke e IPAQ instruments. This study observed two distinct moments of statistical treatment. The first one relates to Data Description according to experimental variables, observing normality evaluation protocols (Kolmogorov-Smirnov) of them and its respective potencies in sample size; above that mean values calculations and other variational dimensions quantification statistical parameters. The second moment related to Inferential Study part searching for testing
the Hypothesis according to Pearson non-parametric Chi-Square to verify the association (p<0.05). The Cluster classification method was also utilized to variables that observe distributions under a normal curve and of parametric continuous content.
Results: From evaluated elderly 67% presented obesity. It is observed that 50% of the evaluated group presents high levels of WC and BHR. This over increasing of fatness accumulated at body central areas is related to higher incidence of degenerative chronicle dysfunctions. The elderly on this study presented total score of 13.17 (57.28%) of satisfaction on WHOQOL-OLD test. This score is mainly due to facets 2, 3, 4 and 6, that supported it. Simultaneously score 1 (sensorial functioning) and score 5
(dying and death) presented the lowest scores. The Cluster group presents a higher prevalence compared to the other (group 2) that possesses a physical activity intermediary score, while group 3 (lower percentile related to others) presented the highest levels of physical activity. When correlating the variables it is observed a significant association to p<0.05 between variables sex and BHR (p=0.017), to subscapular plait with BMI (p=0.006) and WC (p=0.001). A significant association is also observed between variables BHR, Beacke and the total score of quality of life, what represents that individuals with lower values of BHR present higher levels of physical
activity and quality of life.
Conclusion: The global quality of life and its distinct facets seems to be two different ways of evaluating the elderly quality of life: evaluating
quality of life as a whole (total score) or its isolated aspects (facets 1, 2, 3, 4, 5 and 6).
