Introdução: a tuberculose (TB) é uma doença infeciosa, tratável, curável, porém que ainda mesmo em dias atuais continua com altos índices de morbidade e mortalidade. Alguns fatores associados a doença implicam em óbitos dos indivíduos, entre eles o HIV/Aids e outras comorbidades, a multirresistência à medicação, hábitos de vida e fatores socioeconômicos. A internação durante o tratamento pode ser uma necessidade, e poucos estudos abordam sua relação com a mortalidade por TB. Objetivo: realizar revisão sistemática dos fatores associados à sobrevida ou mortalidade e identificar o tempo de sobrevida e fatores associados até o ano de 2016, de doentes internados com TB no município de Manaus no ano 2010; Métodos: foram realizadas uma revisão integrativa de acordo com as recomendações Prisma e análise de sobrevida com dados de uma pesquisa primária ocorrida no ano de 2010. Os dados de internação foram submetidos a um linkage com as bases de dados de mortalidade (SIM), a fim de identificar os óbitos e suas causas. O tempo de sobrevida foi avaliado por meio da taxa de incidência acumulada e executado análise de associação por meio de regressão multivariada, com vistas a identificar os fatores associados à ocorrência de óbito. Resultados: foram apresentados sob a forma de dois artigos científicos, um sobre uma revisão integrativa de mortalidade e sobrevida de doentes com TB, que trouxe fatores como; sexo masculino, idade maior que 30 anos na coinfecção TB/HIV, idade avançada no caso da TB, baixa escolaridade ou até 8 anos de estudo, desemprego, a coinfecção TB /HIV, diagnóstico recente de HIV, células CD4< 50 células/mm3 , doenças imunossupressoras, diabetes, doença renal, tabagismo, alcoolismo e perda de peso como associados à menor sobrevida ou a mortalidade. E o outro artigo contendo os resultados referentes à análise de sobrevivência da coorte de pacientes internados com TB e os fatores associados a menor sobrevida foram: sexo masculino, faixa etária acima dos 50 anos, pouca escolaridade ou sem informação, caquexia, ter 4 pessoas ou mais por quarto. Além disso foi observado que a Aids Influenciou nestes óbitos, mas o doente tendo causa básica TB permanece menos tempo internado e tem pouca sobrevida comparado a outras causas.
Abstract
Knowledge of factors associated with decreased survival and mortality due to tuberculosis (TB) are important for the prevention and proper patient management. The objective was to identify the factors associated with the survival or mortality of patients with tuberculosis, focusing on tuberculosis and not as a result of HIV co-infection, even if it appears in the articles identified. A review of the literature was carried out, whose searches occurred in the databases: PubMed, Web of Science, Scopus, Bireme/BVS and SciELO.Org, with the following descriptors: ("Survival" OR "Mortality") AND ("Epidemiological factors" OR "Epidemiological determinants" OR "Associated factors"). The recommendations of the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyzes) methodology were used. The factors associated with mortality or reduced survival were: male sex, age greater than 30 years in TB/HIV co-infection, advanced age in case of isolated TB, low schooling or up to 8 years of schooling, unemployment, TB/HIV co-infection, recent diagnosis of HIV, CD4<50 cells, other immunosuppressive diseases, diabetes, renal disease, smoking, alcoholism, weight loss, treatment recently started. Factors associated with survival or mortality due to TB do not differ from those associated with morbidity. However, excluding TB/HIV co-infection, socioeconomic factors are evidenced.
