Ao longo de dois mil e quinhentos anos, a relaçäo médico-paciente foi guiada pelos princípios de beneficência e de näo-maleficência, princípios que justificaram o paternalismo médico. A formulaçäo dos direitos do paciente, em especial o direito à autodeterminaçäo, trouxe novos desafios éticos à prática médica. Tem por pressuposto que os princípios bioéticos prima facie de respeito à autonomia e seu correlato, o consentimento livre e esclarecido, têm levado a mudanças substanciais de ética médica tradicional. Tendo por referência a abordagem principialista da bioética, discute a posiçäo em que ocupa esses princípios, em conjunto com os outros propostos pelo principialismo (beneficência, nao-maleficência e justiça), na formulaçäo de reivindicaçöes de cinco associaçöes de doentes crônicos com sede no município do Rio de Janeiro.
Resumen en ingles
During the past two thousand five hundred years, doctor-patient relatioship has been based on the principles of beneficence and non-maleficence, principles wich justified medical paternalism. The elaboration of patient’s rights, especially the right to self-determination, has introduced new ethical challenges to medical practice. Our thesis considers that bioethical prima facie principles regarding respect to patient’s autonomy and its corollary – free informed consent – have led to substantial changes in traditional medical ethics. Having considered bioethics principlism (beneficence, non-maleficence and justice) in the elaboration of demands of five associations of chronic patients located in the city of Rio de Janeiro. The activities of these associations seek the complex task of articulating these principles, replacing traditional ethical values of medical practice which were based upon the absolute principle of beneficence.
