O Sistema Nacional de Auditoria (SNA) foi estabelecido pela Lei 8080, como um mecanismo de controle técnico e financeiro para regular as ações e os serviços de saúde, em todo o território nacional, sob competência do Sistema Único de Saúde (SUS) e em cooperação com os estados, Distrito Federal e municípios. A instituição desse sistema de auditoria, desde então, tem sido considerada como um importante instrumento para a
manutenção e o desenvolvimento do SUS. Passados mais de dez anos da
regulamentação do SNA, o presente estudo pretendeu analisar sua estruturação, seus avanços, desafios, bem como a sua força de trabalho. Adotou-se uma abordagem qualitativa realizando-se grupos focais com os integrantes das equipes de auditoria nas esferas federal, estadual e municipal do SNA, em uma capital da região Sudeste, habilitada como Gestão Plena do Sistema Municipal. Constataram-se vários avanços,
mas, também, desafios das mais diversas ordens. Entre eles, encontram-se: a consolidação da auditoria em saúde como campo de conhecimento e de
profissionalização; a inexistência do cargo de auditor em saúde; e a criação de uma articulação efetiva entre as três esferas gestoras do SUS. Conclui-se que a regulamentação do SNA instituiu um novo campo de produção de conhecimento para tratar das necessidades relativas à prática de auditoria, assim como das experiências daí decorrentes. A descentralização do sistema ainda não resultou em articulações significativas entre as esferas federal, municipal e estadual do SNA, o que vem inviabilizando um correto conhecimento acerca do seu funcionamento. Em relação à força de trabalho desse sistema, observa-se que ela é caracterizada pela
multiprofissionalidade, mas com um predomínio da área biomédica. A inexistência do cargo de auditor em saúde tem sido apontada como impedimento para exercícios adequados da função de auditoria. Portanto, o SNA tem um longo caminho a trilhar para o estabelecimento de um consolidado e dinâmico campo de produção para a manutenção e aprimoramento do SUS, apesar do reconhecimento, por parte dos
pesquisados, da relevância social já conquistada pelo SNA, como âmbito de práticas.
Abstract
The National System of Auditorship (SNA) was established by the Law 8080, as a technical and financial control mechanism to regulate the actions and the health services, in all the domestic territory, under the ability of the Brazilian Health System (Unified Health System - SUS) and in cooperation with the states, the Federal District and the cities. The institution of this system of auditorship has been considered an important instrument to the maintenance and development of the SUS. Almost ten years
after the regulation of the SNA, the present study intended to analyze its structure, advances, challenges, as well as its force of work. A qualitative aproach was adopted, which included the realization of focal groups with the integrants of auditorship teams in the federal, state and municipal spheres of the SNA, in a capital of the Southeastern region, qualified as Full Management of the Municipal System. Some advances and also
challenges of the most diverse orders have been evidenced. Among them we have: the consolidation of the auditorship in health as a knowledge and professionalization field, the inexistence of a health auditor’s position, and the creation of an effective articulation among the three managing spheres of the SUS. The conclusion was that the regulation of the SNA started a new field of knowledge production to deal with necessities related
to the auditorship practice, as well as with the experiences that came from this. The decentralization of the system hasn’t resulted in significant joints between the federal, municipal and state spheres of the SNA yet, what makes the correct knowledge concerning its functioning impracticable. The force of work of this system is characterized by the multiprofissionality but with the predominance of the biomedical area. The inexistence of a health auditor’s position has been pointed as an impediment to the realization of the auditorship proper functions. Therefore, the SNA has a long
way to tread in order to establish a dynamic and consolidated field of production for the maintenance and improvement of the SUS, despite its recognition by the searched people, and the social relevance already conquered by the SNA, as a scope of practices.
