Nesta tese foi analisado, através de um estudo ecológico e de um estudo de base individual, o cumprimento de duas metas da Organização Mundial de Saúde para a cárie dentária no ano 2000: o percentual de crianças livres de cárie aos cinco anos de idade e o CPO-D aos 12 anos de idade.
No estudo ecológico o objetivo foi analisar o cumprimento das referidas metas em 80 municípios do estado de Minas Gerais – Brasil, identificando fatores associados à variação no CPOD médio e na ocorrência de CPOD 3, entre crianças de 12 anos, e no percentual de livres de
cáries aos cinco anos. Foram utilizados dados secundários de diferentes fontes. As técnicas de regressão linear e regressão logística múltiplas foram aplicadas nas análises das variáveis dependentes numéricas e dicotômica consideradas. Indicadores sócio-econômicos e da oferta/utilização de serviços odontológicos foram empregados como variáveis explicativas
potenciais. As metas expressas pelo CPO-D 3 aos 12 anos e pelo percentual mínimo de 50% de crianças livres de cárie aos cinco anos foram atingidas por 37% e 9% dos municípios respectivamente. De modo geral, as variáveis dependentes mostraram-se associadas ao nível sócio
econômico, não se identificando associações com as variáveis relativas à atenção odontológica. Os resultados não devem ser generalizados, mas indicam iniqüidades na saúde bucal e o papel desempenhado por fatores sócio-econômicos e a fluoretação da água.
O estudo de base individual foi desenvolvido em Ipatinga, um município médio de Minas Gerais com alto nível de Desenvolvimento Humano, com água fluoretada e um modelo baseado na prevenção onde o cumprimento de ambas as metas foi alcançado em 2003. Foram realizados
exames clínicos em 232 crianças de cinco anos e 151 de doze anos de idade e um questionário foi aplicado aos pais. Entre as crianças de cinco anos de idade foi encontrada uma associação positiva entre ser livre de cárie e o nível de escolaridade dos pais e associação negativa com o fato de estudar em uma escola pública (associação borderline) ou numa escola filantrópica comparada com uma escola privada e tendo utilizado o sistema público de saúde comparado com outras condições. Por outro lado entre as crianças de doze anos foi encontrada indicação de associação ente o nível de CPO-D, tipo de escola e aplicação de selantes diferenciando o risco de um CPO-D muito alto e muito baixo.
Abstract
In this work we analyzed, through an ecological and an individual data-based study, the accomplishment of two World Health Organization goals for dental decay in 2000: the proportion of decay-free 5-year-old children, and the DMFT among 12-year-old children.
The ecological study was aimed at analyzing the accomplishment of the goals in 80 municipalities of Minas Gerais state, in Southeast Brazil, identifying the factors associated with the variation in the mean DMFT, and with the occurrence of DMFT 3 among 12-year-old children,
and with the proportion of decay-free 5-year-old children. We used secondary data from different sources. Linear regression and logistic regression were employed in considering the focused numeric and dichotomous variables, respectively. Socio-economic and offer/use of dental care indicators were employed as potential explanatory variables. The goals expressed in terms of DMFT 3, among 12-year-old children, and of a minimum of 50% of decay free 5-year-old children, were reached by 37% and 9% of the municipalities respectively. In general, the dependent variables were found to be associated with socio-economic level, and not associated with dental care attention. The results can not be generalized, but point out iniquities in Oral Health, and the role played by socio-economic factors, as well as water fluoridation.
The individual data-based study was conducted in Ipatinga, a medium-size municipality in Minas Gerais, with high Human Development Index level, fluoridated water, and an oral care model focused on preventive procedures, where accomplishment of both goals was observed in 2003. Clinical exams were performed in 232 5-year-old and 151 12-year-old children, and a
questionnaire was applied among their parents. Among 5-year old children, being decay-free was found to be positively associated with the father’s schooling level, and negatively associated with studying in a public (borderline association) or a philanthropic, compared to a private school, and
having utilized public health care, compared to other conditions. On the other hand, among 12-yearold children, we found indication of an association between DMFT level and school type, and the role of sealants in differentiating the risk of a very high versus a very low DMFT.
