A tuberculose (TB) é um grave problema de saúde no Município do Rio de Janeiro. O abandono
do tratamento da TB é um dos grandes desafios para o controle da doença. Esta dissertação teve
por objetivo analisar a relação do abandono do tratamento de TB pulmonar (TBP) e as
condições de vida, a partir de uma abordagem espacial, na Área de Planejamento de Saúde (AP)
3.1 do Município do Rio de Janeiro, no período de 2010 a 2016. Para tal, foi realizado um
estudo descritivo ecológico, com georreferenciamento dos casos de TB, por setor censitário,
utilizando-se a base de dados do Sistema de Informação de Agravos de Notificação (SINAN),
da Secretaria Municipal de Saúde do Rio de Janeiro (SMS-RJ). Foram analisadas variáveis
sociodemográficas e clínico-epidemiológicas dos 7399 casos totais de TBP, dos quais 1495
casos tiveram como desfecho o abandono. Para levantamento das condições de vida da
população da AP 3.1 foi utilizado dados socioeconômicos e o Índice de Desenvolvimento Social
(IDS), baseados em variáveis do Censo de 2010, por setores censitários. Para a dimensão
referente a oferta de serviços de saúde (oferta de teste HIV e existência de TDO) utilizou-se os
dados do SINAN, relativo as unidades de saúde de atenção primária, por bairros. As variáveis
relacionadas ao abandono foram sexo masculino, na faixa etária de 20 a 39 anos, com baixa
escolaridade e com predomínio na raça negra. Em relação à realização do teste HIV, menos de
70% dos casos tiveram o exame realizado, quando o preconizado é de 100%. O percentual de
pacientes que abandonaram fazendo uso de TDO (55,5%) ou não (39,0%), mostrou que a
supervisão não agiu para reduzir o abandono. Na análise espacial por bairros, Manguinhos e
Maré são os que apresentaram altos percentuais de abandono do tratamento de TBP. Na análise
por setores censitários, o Mapa de Calor (Kernel) permitiu visualizar áreas de alta intensidade
da proporção de abandono, dentro dos bairros de Tauá, Pitangueiras, Cacuia, Maré,
Manguinhos, Penha e Cordovil relacionadas a setores censitários do tipo aglomerado
subnormal, com baixo IDS (< 0,531), com alta densidade de pobres, com uma média de mais
de três moradores por domicílios e com analfabetismo de moradores de 10 a 14 anos. Mediante
os resultados deste estudo, há a possibilidade de se redefinir as ações de saúde locais, de forma
equânime e incorporar as ações de georreferenciamento como mais um instrumento de
vigilância e controle epidemiológico.

Tuberculosis (TB) is a serious health problem in the city of Rio de Janeiro. Abandoning TB
treatment is one of the major challenges for disease control. This dissertation aimed to analyze
the relationship between the abandonment of pulmonary TB (TBP) treatment and living
conditions, using a spatial approach in the Rio de Janeiro Health Planning Area (PA) 3.1 from
2010 to 2016. To this end, a descriptive ecological study was carried out, with georeferencing
of TB cases by census tract, using the database of the Disease Notification Information System
(SINAN), of the Rio de Janeiro Municipal Health Secretariat (SMS-RJ). Sociodemographic and
clinical-epidemiological variables of the 7399 cases of TBP were analyzed, of which 1495 had
the abandonment outcome. To survey the living conditions of the population of the AP 3.1,
socioeconomic data and the Social Development Index (SDI) were used, based on 2010 Census
variables, by census tract. For the dimension of health service provision (HIV testing offer and
existence of DOT), SINAN data on primary health care units by neighborhoods were used. The
variables related to the abandonment were being male, aged 20-39 years with low education
and being black. Regarding the HIV test, less than 70% of the cases had the test performed,
when the recommendation is 100%. The percentage of abandonment among patients who used
DOT (55.5%) or not (39.0%) showed that supervision did not act to reduce abandonment. In
the spatial analysis by neighborhoods, Manguinhos and Maré presented the highest dropout
percentages. In the analysis by census tracts, the Heat Map (Kernel) allowed the visualization
of areas of high intensity of abandonment ratio, within the neighborhoods of Tauá, Pitangueiras,
Cacuia, Maré, Manguinhos, Penha and Cordovil. They were related to census tracts of
subnormal cluster type, low IDS (<0.531), with high density of poor, more than three people
per household and illiteracy residents 10-14 years. Through the results of this study, it is
possible to redefine local health actions equitably and incorporate georeferencing actions as
another instrument for surveillance and epidemiological control.
