Os estafilococos coagulase-negativa (ECN) são considerados patógenos potencialmente causadores de infecções no homem, principalmente aquelas relacionadas ao uso de dispositivos médicos. O tratamento destas infecções tem se tornado cada vez mais um desafio para a saúde pública, uma vez que os ECN têm apresentado resistência a múltiplas drogas antimicrobianas. Os desinfetantes de superfícies, como os compostos quaternários de amônio, amplamente usados em ambiente hospitalar, muitas vezes também se tornam ineficientes contra estes patógenos hospitalares o que pode ser evidenciado pelo surgimento de cepas resistentes ou mesmo apresentando susceptibilidade reduzida a estes compostos. Neste estudo, tivemos como objetivos identificar isolados clínicos de ECN oriundos de um hospital público do Rio de Janeiro utilizando-se sistemas automatizados e semi-automatizados de identificação, verificar a susceptibilidade destes isolados clínicos às drogas antimicrobianas fenotipicamente ou genotipicamente e avaliar a susceptibilidade a um desinfetante à base de compostos quaternários de amônio. Como a espécie Staphylococcus epidermidis foi prevalente entre os 63 isolados clínicos estudados, sua diversidade genética foi analisada por PFGE. Apenas um dos isolados clínicos não foi identificado pelos métodos adotados. Dentre os isolados, 69,8por cento apresentaram resistência a mais de seis drogas, representando uma séria preocupação. O método automatizado Vitek detectou uma alta taxa de resistência à oxacilina, que estava associada à resistência intermediária à vancomicina em um isolado clínico. A resistência à oxacilina foi examinada também pelos testes de triagem em ágar contendo oxacilina e de difusão em disco contendo cefoxitina. Comparando estes dois métodos à detecção do gene mecA, considerada como padrão-ouro para indicar isolados meticilina-resistentes, o último mostrou melhor acurácia para revelar a expressão da PBP2a.
