CONTROLE FÍSICO DE BOLSAS DE SANGUE: AVALIAÇÃO RETROSPECTIVA, IMPLEMENTAÇÃO DE NOVOS MÉTODOS E DISCUSSÃO REGULATÓRIA

O Instituto Nacional de Controle de Qualidade em Saúde é um ente federal que oferece suporte às ações de vigilância sanitária. Dentre os produtos avaliados, tem- se as bolsas de sangue que são submetidas na modalidade de análise prévia, conforme estabelecido pela Resolução RDC nº 35 de 2014, da Anvisa. O objetivo do estudo foi discutir o controle da qualidade com enfoque nos ensaios físicos através da avaliação retrospectiva das análises realizadas no Instituto, propor a implementação de novos parâmetros de controle, discutir se estes são imprescindíveis para a qualidade de bolsas de sangue e submetê-los aos compêndios oficiais relacionados ao produto. Na etapa inicial realizou-se a avaliação retrospectiva dos resultados analíticos dos ensaios físicos de permeabilidade ao vapor, pH, volume da solução disponíveis no Sistema de Gerenciamento de Amostras Laboratoriais no período de 2011 a 2017. Esta etapa permitiu traçar o perfil dos produtos registrados no país e deste selecionar as amostras para o estudo de implementação de novos parâmetros de controle pelo Instituto. O presente estudo constatou que as bolsas de sangue disponíveis no mercado nacional apresentam baixos percentuais de perda na avaliação da permeabilidade a vapor tendo em 209 unidades (95,9% das avaliadas) valores inferiores a 0,30%. Os valores de pH se apresentaram dentro dos parâmetros permitidos pela legislação: soluções anticoagulantes ACD-A apresentaram o pH 5,0. Soluções CPD e CPDA, apresentaram pH na faixa de 5,2 a 5,8 e 5,6 a 5,8, respectivamente. As soluções preservadoras SAG-M1 apresentaram maior variabilidade no pH (4,7 a 5,7) e 52 dentre as 53 soluções avaliadas tinham volume de acordo com o preconizado pela RDC nº 35 de 2014. No estudo de implementação do ensaio de partículas subvisíveis foi observado que todas as 28 soluções avaliadas estavam dentro do parâmetro determinado para injetáveis segundo a Farmacopeia Europeia, indicando resultados abaixo da metade do limite estabelecido em 26 das quantificadas, o que representou 92,9% das soluções. Os resultados analíticos para o ensaio de absorção UV indicaram que dez amostras estavam dentro do parâmetro de aceitação proposto pela Farmacopeia Europeia (2017) e três apresentaram sinais de absorbância em 229,1nm; 228,0nm e 229,1nm, comprimentos de onda bem próximos à 230nm, região preconizada para o ensaio, sendo consideradas insatisfatórias. Para o ensaio de transparência todas as amostras analisadas foram consideradas satisfatórias, tanto para análise visual quanto para a análise espectrofotométrica. Além disso, os resultados do ensaio de pH e de partículas subvisíveis poderão auxiliar estudos preliminares à publicação de novas monografias relacionadas. O presente trabalho demonstra a atuação do Instituto como ente de extrema importância científica, regulatória e política dentro do cenário de vigilância sanitária do Brasil.

The National Institute of Quality Control in Health is a federal body that supports health surveillance actions. Among the products evaluated, there are the blood bags that are submitted in the prior analysis modality, as established by Resolution RDC No 35 of 2014, of Anvisa. The purpose of the study was to discuss quality control with a focus on physical tests through the retrospective evaluation of the analyzes performed at the Institute, propose the implementation of new control parameters, discuss whether these are essential for the quality of blood bags and submit them to the official compendiums related to the product. In the initial stage, a retrospective evaluation of the physical results of the vapor permeability, pH and solution volume tests available in the Laboratory Samples Management System was performed in the period from 2011 to 2017. This stage allowed us to trace the profile of the products registered in the country and from this select the samples for the study of the implementation of new control parameters by the Institute. The present study found that the pockets of blood available in the national market have low values of vapor permeability having 209 units (95.9% of the evaluated ones) values lower than 0.30%. The pH values were within the parameters allowed by the legislation: ACD-A anticoagulant solutions presented pH 5.0. CPD and CPDA solutions presented pH in the range of 5.2 to 5.8 and 5.6 to 5.8, respectively. The SAG-M1 preservation solutions presented higher pH variability (4.7 to 5.7) and 52 of the 53 solutions evaluated had volume according to the one recommended by the RDC nº 35 of 2014. In the study of implementation of the test of subvisible particles it was observed that all 28 evaluated solutions were within the parameter determined for injectables according to the European Pharmacopoeia, indicating results below half of the limit established in 26 of the quantified ones, which represented 92.9% of the solutions. The analytical results for the UV absorption assay indicated that ten samples were within the acceptance parameter proposed by the European Pharmacopoeia (2017) and three showed absorbance signals at 229.1 nm; 228.0nm and 229.1nm, wavelengths very close to 230nm, region recommended for the test, being considered unsatisfactory. For the transparency test all analyzed samples were considered satisfactory, both for visual analysis and for spectrophotometric analysis. In addition, the pH and subvisible particle test results may support preliminary studies for the publication of new related monographs. The present work demonstrates the Institute's performance as an entity of extreme scientific, regulatory and political importance within the scenario of sanitary surveillance in Brazil.
