A urbanização vivenciada pela população brasileira, a partir da década de 1970,
contribuiu para a conformação de um processo de transição epidemiológica, com a
transferência de perfis de morbi-mortalidade característicos do meio rural para o
ambiente urbano. Neste contexto, encaixa-se a leishmaniose visceral (LV) e seu
processo de urbanização. Em Pernambuco, o panorama da LV não é diferente.
Observa-se a persistência de velhas áreas de ocorrência e surgem novos focos,
estando presente em todas as regiões do seu território, fato que aponta para a
necessidade de estudar a epidemiologia da LV nos municípios do estado, de forma a
apreender diferenciais que expressem o risco de adoecer. Para tanto, este estudo foi
constituído pelo universo dos casos novos de LV, residentes no estado de
Pernambuco, no período entre 2000 e 2006. O trabalho utilizou o Sistema de
Informações sobre Agravos de Notificação (Sinan) e o Censo Demográfico 2000
(IBGE), e construiu a partir de informações sócio-econômicas, um indicador de
situação coletiva de risco para a ocorrência da LV em Pernambuco. Observou-se
que dos 1.189 casos confirmados notificados, 1.008 eram casos novos, não mais
restritos a áreas rurais ou interior do Estado, mas também em centros urbanos. O
indicador de carência social demonstrou, apesar de pequena, sensibilidade na
predição de áreas de risco, pois os maiores coeficientes de detecção situaram-se no
estrato de alto risco (piores condições sociais). Este resultado aponta a utilização do
indicador proposto como ferramenta complementar ao planejamento de ações para
o controle da LV. É preciso estimular mais estudos para identificação dos fatores de
risco para LV, bem como defender a qualidade dos dados do Sinan. Sabe-se da
necessidade de evitar ou minimizar a expansão da doença, e para isso é
fundamental incorporar novas metodologias que contribuam para o conhecimento do
seu comportamento nas populações, subsidiando ações eficazes de controle e
prevenção, localizando grupos prioritários em espaços determinados e assim
auxiliando no planejamento das ações de controle da doença.
Resumen en ingles
The urbanization experienced by the Brazilian population, since the decade of 1970,
contributed to the conformation of a process of epidemiological transition, with the
transfer of morbidity and mortality profiles characteristic of the rural environment to
the urban environment. In this context, embedding itself to Visceral Leishmaniasis
(VL) and the process of urbanization. In Pernambuco, the panorama of the VL is not
different. While observing the persistence of old areas of occurrence there are new
outbreaks, being present in all areas of its territory, a fact that points out the need to
study its epidemiology in the cities of the state, in order to seize differentials which
express the risk of becoming ill. Thus, this study was built with the universe of new
VL cases, inhabiting the state of Pernambuco, in the period between 2000 and 2006.
The work used the Sistema de Informações sobre Agravos de Notificação (Sinan)
and the Demographic Census 2000 (IBGE), and built from socio-economic
informations, an indicator of collective situation risk for the occurrence of VL in
Pernambuco. It was observed that of the 1,189 confirmed cases reported, 1,008
were new cases, no longer restricted to rural areas or countryside but also in urban
centers. The social deprivation indicator demonstrated, although small, sensitivity in
predicting risk areas, because the higher coefficients of detection were in the stratum
of high risk (worst social conditions). This result suggests the use of the indicator
proposed as a complementary tool for planning actions to control the VL. We must
stimulate further studies to identify risk factors for the VL, as well as defend the
quality of the data of Sinan. The need to prevent or minimize the spread of the
disease is known, and it is essential to incorporate new methodologies that contribute
to the knowledge of their behavior in the population, subsidizing effective control and
prevention actions, locating priority groups in certain areas and thus helping in the
planning of actions to control the disease.
